摘 要:目的通过CBCT评估不同垂直骨面型成年患者颧牙槽嵴骨骼特征,并分析微种植体的植入风险。方法选取2016—2020年就诊于南京医科大学附属口腔医院正畸科的67例成年患者的CBCT数据,按照垂直骨面型分为三类。在冠状面上确定左侧第一磨牙近中颊根,第一磨牙远中颊根及第一磨牙远中颊根和第二磨牙近中颊根之间这三个测量截面,分别测量三个截面上与上颌后牙牙合平面成65°和70°处颧牙槽嵴的有效骨量。结果颧牙槽嵴处测得的有效骨量的测量平均值自第一磨牙近中颊根向后至第一磨牙远中颊根和第二磨牙近中颊根之间逐渐减小,但差异无统计学意义。高角组,均角组和低角组患者在第一磨牙近中颊根、第一磨牙远中颊根及第一磨牙远中颊根和第二磨牙近中颊根之间这三个测量截面处测得的有效骨量均无统计学差异。结论上颌颧牙槽嵴区植入微种植体前应拍摄CBCT,宜选用6 mm短种植体于第一磨牙近中颊根处,在根尖水平与牙合平面成70°植入以减少植入后穿通上颌窦的风险。Objective To evaluate bone thickness of the infrazygomatic crest(IC)in adults with different vertical skeletal patterns in order to analyze risks for mini-screws insertion.Methods CBCT of 67 adult patients with different vertical skeletal patterns who visited Affiliated Stomatological Hospital of Nanjing Medical University from 2016 to 2020 were collected and divided into three groups according to vertical skeletal patterns.The bone thickness of the IC was evaluated at 65°and 70°angulations on the coronal slice in the mesial buccal root(MB)and distal buccal root(DB)of the maxillary left first molar and the region between the first and second molars(PR)on the left side.Results Bone thickness of the IC decreased from the mesial buccal root of the maxillary first molar to the region between the first and second molars.There were no statistical difference of bone thickness in IC between different skeletal patterns in the three selected measurement planes.Conclusion CBCT may be necessary to install mini-screws in the IC correctly.Six mm mini-screws should be selected.The best site to install mini-screws is the first molar mesial buccal root and at 70°angulation which will reduce the risk of invading the maxillary sinus.
