活血通结解痉方随症加减对三叉神经痛患者临床症候及VIP、β-EP水平的影响

摘　　要：目的:观察活血通结解痉方随症加减对三叉神经痛患者临床症状及血管活性肠肽(VIP)、β内啡肽(β-EP)水平的影响。方法:选取我院2018年7月~2020年7月92例三叉神经痛患者,经随机数字表法分为观察组和对照组,均为46例,对照组采用卡马西平治疗,观察组联合使用活血通结解痉方随症加减治疗。比较两组临床疗效、症状改善情况、治疗前后疼痛情况、炎症因子水平、VIP、β-EP及5-羟色胺(5-HT)水平。结果:两组临床治疗总有效率分别为93.48%、78.26%,观察组高于对照组(P<0.05);观察组治疗后中医证候积分、疼痛每次发作持续时间、发作次数均低于对照组(P<0.05);观察组治疗后VAS评分、PRI评分、PPI评分均低于对照组(P<0.05);观察组治疗后IL-1β、TNF-α、IL-6水平均低于对照组(P<0.05);观察组治疗后VIP水平低于对照组,β-EP、5-HT水平高于对照组(P<0.05)。结论:活血通结解痉方随症加减治疗能提升三叉神经痛患者的临床疗效,缓解疼痛及临床症状,改善炎症因子水平,调节VIP、β-EP及5-HT水平。Objective: To explore the effect of Modified Huoxue Tongjie Jiejing Recipe on clinical symptoms, levels of vasoactive intestinal peptide(VIP) and β-endorphin(β-EP) for patients with trigeminal neuralgia.Methods: A total of 92 patients with trigeminal neuralgia in the hospital from July 2018 to July 2020 were enrolled and divided into an observation group and a controlled group by the random number table method, 46 cases in each group.The controlled group was treated with Carbamazepine, while the observation group was additionally treated with Modified Huoxue Tongjie Jiejing Recipe.The clinical curative effect, symptoms improvement, pain status, levels of inflammatory factors, VIP,β-EP and 5-hydroxytryptamine(5-HT) before and after treatment were compared between the two groups.Results: The total response rate of clinical treatment in the observation group was higher than that in the controlled group(93.48% vs 78.26%)(P<0.05).After treatment, scores of TCM syndromes, duration and times of pain onset in the observation group were lower than those in the controlled group(P<0.05),VAS,PRI and PPI scores were lower than those in the controlled group(P<0.05),levels of IL-1β,TNF-α and IL-6 were lower than those in the controlled group(P<0.05),VIP level was lower than that in the controlled group, and levels of β-EP and 5-HT were higher than those in the controlled group(P<0.05).Conclusion: Modified Huoxue Tongjie Jiejing Recipe can improve the clinical curative effect for patients with trigeminal neuralgia, relieve clinical symptoms and pain, improve levels of inflammatory factors, and regulate levels of VIP,β-EP and 5-HT.
