锁孔夹闭手术对破裂脑动脉瘤患者术后NF-κB、VEGF水平及脑血管痉挛的影响研究

摘　　要：目的研究将锁孔夹闭手术应用于破裂脑动脉瘤患者中的作用与效果。方法2017年10月至2019年12月于我院接收并对其进行治疗的破裂脑动脉瘤患者共72例,根据不同治疗方式划分组别,对照组36例行大骨瓣开颅手术,观察组36例行锁孔夹闭手术,比较两组应用效果。结果术后第3天、术后第7天,观察组的血清核因子κB(NF-κB)、血管内皮生长因子(VEGF)显著低于对照组,P<0.05;术后第7天,观察组的脑血管痉挛的总发生率显著低于对照组,治疗总有效率显著高于对照组,P<0.05;对比对照组,观察组的手术时间、术中出血量、住院时间等手术相关指标均更优,P<0.05。结论破裂脑动脉瘤治疗工作中应用锁孔夹闭手术能够改善患者NF-κB、VEGF、各项手术指标,并降低其脑血管痉挛的总发生率,提升其治疗疗效,能够在临床脑动脉瘤的治疗方面给予更多的参考价值。Objective To study the effects and functions of keyhole clipping in patients with ruptured cerebral aneurysm.Methods From October 2017 to December 2019,a total of 72 patients with ruptured cerebral aneurysm received and treated in our hospital were divided into different groups according to different treatment methods.36 patients underwent large craniotomy as the control group and 36 patients underwent keyhole cliping as the observation group.The effects of the two groups were compared.Results On the 3rd and 7th day after the operation,the serum nuclear factor-κB(NF-κB)and vascular endothelial growth factor(VEGF)of the observation group were significantly lower than those of the control group,P<0.05;on the 7th day after the operation,the observation group The total incidence of cerebral vasospasm was significantly lower than that of the control group,and the total effective rate of treatment was significantly higher than that of the control group,P<0.05;compared with the control group,the observation group's operation time,intraoperative blood loss,hospital stay and other related indicators of operation were all Better,P<0.05.Conclusion Keyhole clipping surgery in the treatment of ruptured cerebral aneurysm can improve patients NF-κB,VEGF and various surgical indicators,reduce the total incidence of cerebral vasospasm,and improve its therapeutic effect.It can provide more reference for the treatment of clinical cerebral aneurysm,and can be applied and promoted.
