摘 要:目的观察筋针结合关节松动术对膝骨关节炎兔膝关节软骨中Notch信号通路Notch2、Jagged-1表达的影响,探讨筋针结合关节松动术治疗膝骨关节炎的可行性和相关作用机制。方法选取26只大耳白兔,随机取6只作为空白组,其余兔采用改良Hulth手术法建立左膝膝骨关节炎模型。将建模成功的18只兔随机分为模型组、氨糖组和筋针结合松动术组各6只,氨糖组给予盐酸氨基葡萄糖灌胃6周,筋针结合松动术组给予筋针结合关节松动术治疗6周,空白组和模型组不予任何干预。干预结束后取各组兔的左膝软骨,分别采用Western blot法和荧光PCR法检测软骨细胞中Notch2、Jagged-1的蛋白和mRNA表达量。结果模型组软骨细胞中Notch2、Jagged-1蛋白和mRNA表达量均明显高于空白组(P均<0.05),氨糖组和筋针结合松动术组均明显低于模型组(P均<0.05),且筋针结合松动术组均明显低于氨糖组(P均<0.05)。结论筋针结合关节松动术可以有效下调膝骨关节炎兔软骨中Notch2和Jagged-1的表达,从而保护关节软骨。Objective It is to observe the effect of sinew acupuncture combined with joint mobilization on the expression of Notch2 and Jagged-1 of Notch signal pathway in the cartilage of rabbits with knee osteoarthritis,and to explore the feasibility and related mechanism of the combined sinew acupuncture with joint mobilization in the treatment of knee osteoarthritis.Methods Twenty-six white rabbits with big ears were selected,in which 6 rabbits were randomly selected as the blank group.The remaining rabbits were used to establish the left knee osteoarthritis models by the modified Hulth operation method.The 18 rabbits that were successfully modeled were randomly divided into model group,amino sugar group and combination group,each group had 6 rabbits.The amino sugar group was treated with glucosamine hydrochloride by gavage for 6 weeks,and the combination group was treated with sinew acupuncture combined with joint mobilization for 6 weeks,and no intervention was given to the blank group and model group.After the treatment,the left knee cartilage of the rabbits in each group was taken to detect the protein and mRNA expression of Notch2 and Jagged-1 in chondrocytes by Western blot and fluorescent PCR methods.Results The expression levels of Notch2,Jagged-1 protein and mRNA in chondrocytes in the model group were significantly higher than those in the blank group(all P<0.05),and these indexes in the amino sugar group and the combination group were significantly lower than those in the model group(all P<0.05),and the combination group was significantly lower than the amino sugar group(all P<0.05).Conclusion The combination of sinew acupuncture and joint mobilization can effectively reduce the expression levels of Notch2 and Jagged-1 in the cartilage of the rabbit model of knee osteoarthritis,thus to protect the articular cartilage.
