摘  要:目的探讨食管鳞状细胞癌(ESCC)组织miR-199a-3p、腺苷酸激酶(AK4)的表达水平及临床意义。方法收集2016年1月至2018年6月本院“内镜粘膜下剥离术(ESD)”留存的ESCC组织标本72例及对应癌旁组织标本(距离ESCC组织5cm以上)72例,实时荧光定量PCR(qRT-PCR)法检测ESCC组织和癌旁组织中miR-199a-3p、AK4 mRNA的表达;免疫组织化学法检测ESCC组织中蛋白AK4的表达;对患者术后进行随访,随访时间为3年,记录患者总生存时间(OS);χ^(2)检验分析ESCC组织中miR-199a-3p、AK4的表达与患者临床病理特征间的关系;Spearman法分析ESCC组织中miR-199a-3p、AK4的相关性;Kaplan-Meier生存曲线分析miR-199a-3p、AK4与ESCC患者预后之间的关系。COX回归分析影响ESCC患者预后的危险因素。结果与癌旁组织比较,ESCC组织中miR-199a-3p表达显著降低,AK4 mRNA表达和蛋白表达阳性率显著升高(P<0.05);不同浸润深度ESCC组织miR-199a-3p、AK4表达差异具有统计学意义(P<0.05);ESCC组织中,miR-199a-3p与AK4表达呈负相关(r=-0.442,P<0.05);miR-199a-3p低表达组、AK4阳性表达组3年OS分别低于高表达组和阴性表达组;多因素COX回归分析结果表明miR-199a-3p低表达和AK4阳性表达是ESCC患者的独立预后因素(P<0.05)。结论miR-199a-3p、AK4可能与ESCC的发生发展和预后有关。Objective To investigate the expression levels and clinical significance of miR-199a-3p and adenylate kinase 4(AK4)in esophageal squamous cell carcinoma(ESCC)tissue.Methods Seventy-two ESCC tissue specimens preserved during endoscopic mucosal dissection in our hospital from January 2016 to June 2018 were collected,and 72 adjacent tissue specimens(more than 5 cm away from ESCC tissue)were used as Control.Real-time quantitative PCR(qRT-PCR)was used to detect the expression of miR-199a-3p and AK4 mRNA;immunohistochemical method was used to detect the expression of AK4 protein;The patients after surgery were followed up for 3 years,and the overall survival time(OS)of the patients was recorded;the relationship between the expression of miR-199a-3p and AK4 in ESCC tissues and the clinical pathological characteristics of patients was analyzed;Spearman method was used to analyze the correlation of miR-199a-3p and AK4 in ESCC;Kaplan-Meier survival curve was used to analyze the relationship between miR-199a-3p,AK4 and the prognosis of ESCC patients.COX regression was used to analyze the risk factors affecting the prognosis of ESCC patients.Results Compared with adjacent tissues,the miR-199a-3p expression in ESCC was significantly reduced,and the AK4 mRNA expression and protein expression positive rate were increased(P<0.05);the expression of miR-199a-3p and AK4 in ESCC tissues with different TNM stages and depth of invasion was statistically significant(P<0.05);in ESCC tissues,miR-199a-3p was negatively correlated with AK4 expression(r=-0.442,P<0.05);the 3-year OS of miR-199a-3p-low expression group and AK4-positive expression group was lower than that of miR-199a-3p-high expression and AK4-negative expression group,respectively;multivariate COX regression analysis showed that low expression of miR-199a-3p and positive expression of AK4 were independent prognostic factors in ESCC patients(P<0.05).Conclusion MiR-199a-3p and AK4 may be related to the occurrence,development and prognosis of ESCC.
