摘  要:目的:研究用替诺福韦酯阻断乙型肝炎病毒(HBV)母婴传播的效果。方法:选择2018年3月至2021年1月英德市人民医院收治的50例携带HBV的孕产妇作为研究对象。按照随机数表法将其分为对照组和研究组。用替诺福韦酯对研究组孕产妇进行治疗,不为对照组孕产妇使用任何药物,待两组孕产妇分娩后为其新生儿接种HBV疫苗及注射乙型肝炎人免疫球蛋白。比较两组孕产妇血清HBV-DNA、丙氨酸氨基转移酶的水平及其所产新生儿血清HBV-DNA、乙型肝炎表面抗原的阳性率。结果:治疗4周后及分娩时,研究组孕产妇血清HBV-DNA和丙氨酸氨基转移酶的水平均低于对照组孕产妇,差异有统计学意义(P<0.05)。研究组孕产妇所产新生儿在出生时、出生后3个月及出生后6个月其HBV-DNA和乙型肝炎表面抗原的阳性率均低于对照组孕产妇所产的新生儿,差异有统计学意义(P<0.05)。结论:用替诺福韦酯阻断HBV母婴传播的效果显著,能有效降低新生儿HBV-DNA和乙型肝炎表面抗原的阳性率。Objective:To study the effect of blocking mother-to-child transmission of hepatitis B virus(HBV)with tenofovir dipivoxil.Methods:A total of 50 pregnant women with HBV who were admitted to People’s Hospital of Yingde city from March 2018 to January 2021 were selected as the research object.They were divided into control group and study group according to random number table method.The pregnant women in the study group were treated with tenofovir dipivoxil,and no drugs were used for the pregnant women in the control group.After delivery,the newborns in the two groups were inoculated with HBV vaccine and injected with hepatitis B human immunoglobulin.The levels of serum HBV-DNA and alanine aminotransferase and the positive rates of serum HBV-DNA and hepatitis B surface antigen in newborns were compared between the two groups.Results:After 4 weeks of treatment and delivery,the levels of serum HBV-DNA and alanine aminotransferase in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).The positive rates of HBVDNA and HEPATITIS B surface antigen at birth,3 months after birth and 6 months after birth in the study group were lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion:Tenofovir dipivoxil has a significant effect on blocking mother-to-child transmission of HBV,and can effectively reduce the positive rate of HBV-DNA and hepatitis B surface antigen in neonates.
