胃底静脉曲张用钛夹与尼龙绳辅助的效果分析

摘　　要：目的探讨胃底静脉曲张(IGV1型及GOV2型)内镜下钛夹夹闭辅助与尼龙绳圈套辅助治疗的临床价值。方法回顾性分析2018年6月—2019年12月我院肝硬化合并胃静脉曲张(IGV1型及GOV2型)出血患者36例,其中行钛夹封闭后硬化剂联合组织胶治疗者有20例,另有16例给予胃底静脉曲张尼龙绳圈套后再行硬化剂联合组织胶治疗,评估手术治疗时间、术后再次出血、静脉曲张消失及术后栓塞发生的情况。结果钛夹组较尼龙绳组手术时间短,静脉曲张再出血发生少,且术后栓塞的发生少于尼龙绳组。详细见文章内容。结论钛夹辅助治疗相对于尼龙绳辅助内镜下治疗需要时间短,术后栓塞及出血的发生少,钛夹效果可能优于尼龙绳,但本文样本较少,值得进一步临床研究。Objective To explore the clinical value of endoscopic titanium clip and nylon rope snare in the treatment of gastric fundus varicose veins(IGV1 and GOV2).Methods A retrospective analysis of 36 patients with liver cirrhosis and gastric varices(IGV1 and GOV2)bleeding in our hospital from June 2018 to December 2019.Among them,20 patients were treated with sclerotherapy combined with tissue glue after titanium clip closure Another 16 cases were treated with sclerosing agent combined with tissue glue after receiving a nylon rope snare for gastric varices.The time of surgical treatment,postoperative bleeding,disappearance of varicose veins,and postoperative embolism were evaluated.Results Compared with the nylon rope group,the titanium clip group had shorter operation time,less varicose vein bleeding,and less postoperative embolism than the nylon rope group.Conclusion Compared with nylon cord-assisted endoscopic treatment,titanium clip adjuvant therapy requires shorter time,less postoperative embolism and bleeding,and the effect of titanium clip may be better than nylon cord.However,there are fewer samples in this article and it is worth further clinical research.
