内侧楔骨远端倾斜与拇外翻关系的相关性

摘　　要：目的通过直接测量所选定的足负重正位X线片上内侧楔骨远端倾斜的相关角度,研究其与拇外翻之间的相关关系。方法回顾性选取了中国人民解放军联勤保障部队第九二〇医院2018年1月至2021年2月期间影像科拍摄的足负重正位片,测量所有样本HVA并根据其大小将所有样本分为正常组、轻中度拇外翻组和重度拇外翻组,再测量每个样本的IMA、MAA、MCA、DMCA等,最后对2组的测量指标进行统计分析。结果共收集679例足负重正位片,左足330例(49%)、右足349例(51%),男足262例(39%),女足417例(61%);总患者数为538例,其中男性222例(41%),女性316例(59%);患者平均年龄38(18~82)岁,其中男性平均年龄28(18~73)岁、女性平均年龄42(18~82)岁。轻中度拇外翻组和重度拇外翻组中IMA、MAA、MCA均大于正常组,差异有统计学意义(P<0.05),且重度拇外翻组IMA和MCA均大于轻中度拇外翻组,差异有统计学意义(P<0.05);而DMCA在轻中度拇外翻组和重度拇外翻组中均与正常组之间无统计学差异(P>0.05)。结论内侧楔骨远端关节面倾斜与拇外翻的形成有一定关系,其中MCA与拇外翻之间存在关系、而DMCA与拇外翻的关系则需要更近一步的研究。Objective To investigate the relationship between hallux valgus and the inclination angle of the medial cuneiform obliquity which is measured on the selected feet weight-bearing anteroposterior position X-ray films.Methods A total of 679 cases of AP X-ray of feet weight-bearing taken by the Department of Imaging in our hospital were selected retrospectively from January 2018 to February 2021 and were divided into the normal group,the mild-moderate and the severe bunion group according to their HVA samples measured.After that,the IMA,MAA,MCA,DMCA in each sample were measured and the measurement indexes of the three groups were statistically analyzed.Results Among the 679 cases,330 were left feet(49%),349 were right feet(51%);262 were male feet(39%),and 417 were female feet(61%).The total number of patients was 538,including 222 males(41%)and 316 females(59%).The average age were 38 years(18 to 82 years),including 18 to 73 years of age(average,28 years)for males and 18 to 82 years of age(average,42 years)for females.Both of IMA,MAA and MCA in the mild-moderate and severe bunion group were significantly higher than those in the normal group(P<0.05),and the IMA and MCA in the severe bunion group were significantly higher than those in the mildmoderate bunion group(P<0.05).There was no significant difference of the DMCA in the mild-moderate or the severe bunion group than those in the normal group(P>0.05).Conclusion The obliquity of the distal articular surface of the medial Cuneiform is related to the formation of hallux valgus to some extent,MCA is related to hallux valgus,and the relationship between DMCA and hallux valgus needs further study.
