地塞米松玻璃体内植入物治疗非感染性葡萄膜炎的效果观察

摘　　要：目的观察地塞米松玻璃体内植入物治疗非感染性葡萄膜炎的效果方法回顾性病例系列研究分析2018年10月至2021年5月大连大学附属中山医院非感染性葡萄膜炎接受地塞米松玻璃体内植入物治疗的26例(26只眼)的临床资料。患者年龄24〜91(53.50±16.31)岁,病程(5.73±2.86)d。随访1〜6个月,观察治疗前后的视力、黄斑中心区厚度(CMT)及并发症。结果注药后3周、1、3及6个月,视力(BCVA,logMAR)分别为0.57±0.44、0.43±0.29、0.30±0.25、0.30±0.26均较治疗前提高(t=2.342、3.260、4.070、4.060,均P<0.05)注药后3周、1、3及6个月CMT较治疗前明显降低(t=5.035、5.545、5.940、5.958,均P<0.05)。注药后4只眼眼压升高,4只眼发生白内障,10只眼结膜下出血,对症治疗后均好转。所有患者术后无感染性眼内炎等并发症发生。结论玻璃体内地塞米松植入物治疗非感染性葡萄膜炎安全有效,可提高视力,降低CMT.Objective To observe the efficacy of dexamethasone intravitreal implant(DEX)in the treatment of noninfectious uveitis.Methods This was a retrospective case series study.The clinical data of 26 eyes of 26 patients diagnosed with noninfectious uveitis from Oct.2018 to May 2021 in Zhongshan Hospital Affiliated to Dalian University were retrospectively analysed.The age of patients was 24-91(53.50±16.31)years.The course of disease was(5.73±2.86)days.All cases received intravitreal implant of dexamethasone.The patients were followed up for 6 months.The visual acuity,central macular thickness(CMT)and complications before and after treatment were observed.Results Tlie visual acuity(BCVA,logMAR)were 0.57±0.44,0.43±0.29,0.30±0.25,0.30±0.26 at 3 weeks,1,3 ami 6 months after injection,which were significantly increased compared with baseline before injection(t=2.342,3.260,4.070,4.060;all P<0.05).The CMT significantly decreased at 3 weeks,1,3 and 6 months after injection compared with baseline(t=5.035,5.545,5.940,5.958;all P<0.05).There were 4 eyes with elevated intraocular pressure,4 with cataract and 10 eyes with sulxonjunctival hemorrhage,with through symptomatic treatment were in better condition.There were no postoperative complication such as infections endophthalmitis.Conclusion Dexamethasone intravitreal implant is effective and safe for tlie treatment of noninfectious uveitis,which can improve visual ac uity and reduce CMT.
