摘 要:目的探讨β_(3)肾上腺素能受体(β_(3)-AR)基因Trp64Arg多态性与高血压患者发生心脑血管事件的相关性。方法选择首都医科大学宣武医院心脏内科及河北省人民医院心血管内科确诊为原发性高血压(EH)无心血管疾病的门诊患者755例,根据基因分型分为Trp/Trp(TT)组282例,Trp/Arg(TA)组361例,Arg/Arg(AA)组112例。记录主要不良心脑血管事件(MACCE)。结果3组MACCE发生率比较,有统计学差异(P=0.006)。Kaplan-Meier曲线分析显示,AA组MACCE发生率明显高于TA组和TT组(P_(log rank)=0.006)。β_(3)-AR基因AA基因型对MACCE、冠状动脉疾病、脑卒中有显著的独立预测作用(95%CI:1.322~3.175,P=0.001;95%CI:1.050~2.930,P=0.032;95%CI:1.238~5.408,P=0.011)。结论高血压患者β_(3)-AR基因AA变异对MACCE、冠状动脉疾病和脑卒中的发生具有显著的独立预测作用,与其他已知的心脑血管危险因素无关。Objective To study the relationship between β_(3)-AR gene Trp64Arg polymorphism and MACCE in EH patients.Methods Seven hundred and fifty-five essential hypertension patients without cardiovascular disease admitted to Affiliated Xuanwu Hospital of Capital Medical Univer-sity and Hebei Provincial People's Hospital were divided into TT group(n=282),TA group(n=361)and AA group(n=112).MACCE were their main end point.Results The incidence of MACCE was significantly lower in TT group than in TA and AA groups(P=0.006).Kaplan-Meier curve analysis showed that the incidence of MACCE was significantly higher in AA group than in TA group and TT group(P_(log rank)=0.006).The AA genotype of β_(3)-AR gene was an inde-pendent predictor for the incidence of MACCE,CAD and cerebral stroke(95%CI:1.322-3.175,P=0.001;95%CI:1.050-2.930,P=0.032;95%CI:1.238-5.408,P=0.011).Conclusion AA genotype variation of β_(3)-AR gene is an independent predictor for MACCE,CAD and cerebral stroke in EH patients and is not related with the other known cardiovascular risk factors.
