摘　　要：目的探讨皮质基底节综合征(CBS)的临床及影像学特征。方法选择2017年7月~2021年4月我院神经内科门诊和病房就诊的CBS患者20例,依据^(18)F氟脱氧葡萄糖(^(18)F-FDG)正电子发射断层显像(PET)的脑代谢模式分为阿尔茨海默病(AD)源性CBS(CBS-AD)组8例和非CBS-AD组12例,用简易智能状态检查量表(MMSE)评估认知功能状态,采用内侧颞叶萎缩(MTA)视觉评分评估患者海马萎缩情况,回顾性分析2组临床资料。结果CBS-AD组MMSE评分显著低于非CBS-AD组[(13.7±6.0)分vs(24.3±4.9)分,P<0.01],MTA异常比例高于非CBS-AD组(75.0%vs 16.7%,P<0.05)。首发症状中,与非CBS-AD组比较,CBS-AD组记忆力减退比例更高(75.0%vs 0,P<0.01);在病程中,与非CBS-AD组比较,CBS-AD组记忆力减退比例明显升高(100.0%vs 25.0%,P<0.01)。结论^(18)F-FDG PET脑代谢减低模式可能有助于CBS-AD与非CBS-AD的鉴别诊断。Objective To study the clinical and imaging characteristics of corticobasal syndrome(CBS).Methods Twenty CBS patients admitted to our hospital were divited into CBS-AD group(n=8)and non CBS-AD group(n=12)according to the ^(18)F-FDG PET-detected brain metabolism pattern.The cognitive function in two groups was assessed according to their MMSE scale score and hippocampal atrophy in two groups was evaluated according to their medial temporal atrophy(MTA)score.The clinical data of CBS patients were retrospectively analyzed.Results The MMSE scale score was significantly lower while the abnormal MTA ratio was significantly higher in CBS-AD group than in non CBS-AD group(13.7±6.0 vs 24.3±4.9,P<0.01;75.0%vs 16.7%,P<0.05).The incidence of hypomnesia in first-ever symptoms and in CBS course was significantly higher in CBS-AD group than in non CBS-AD group(75.0%vs 0;100.0%vs 25.0%,P<0.01).Conclusion FDG-FDG PET-detected brain metabolism reduction pattern in CBS patients contributes to the differential diagnosis of CBS-AD and non CBS-AD.
