摘　　要：目的观察胃癌患者组织中上皮间质转化(EMT)相关蛋白、转化生长因子(TGF)-β1 mRNA表达与淋巴结转移的相关性。方法将该院2016年1月至2020年11月收治的51例胃癌合并淋巴结转移患者纳入淋巴结转移组,另将该院同期收治的51例胃癌未合并淋巴结转移患者纳入无淋巴结转移组,收集患者资料,并进行回顾性分析。采用回归分析检验患者肿瘤分化程度,TNM分期,脉管内癌栓,EMT相关蛋白[E-钙黏蛋白(E-cadherin)、波形蛋白(Vimentin)、β-链蛋白(β-catenin)],TGF-β1 mRNA相对表达水平与淋巴结转移的关系,并分析患者胃癌组织中EMT相关蛋白、TGF-β1 mRNA相对表达水平对淋巴结转移的预测效能。结果初步比较两组的基线资料后,经回归分析检验结果显示,胃癌患者肿瘤分化情况、病理分期、脉管内癌栓情况、胃癌组织内E-cadherin表达、Vimentin、β-catenin及TGF-β1 mRNA相对表达水平均与患者淋巴结转移有关,肿瘤低分化、TNM分期Ⅲ~Ⅳ期、有脉管内癌栓,胃癌组织中E-cadherin相对表达水平降低,Vimentin、β-catenin及TGF-β1 mRNA相对表达水平升高均可能是胃癌患者淋巴结转移的风险因子(P<0.05);绘制受试者工作特征曲线发现,胃癌组织中E-cadherin、Vimentin、β-catenin、TGF-β1 mRNA相对表达水平预测淋巴结转移的曲线下面积均>0.800,均有一定预测效能。结论胃癌患者组织中EMT相关蛋白、TGF-β1 mRNA相对表达水平与淋巴结转移密切相关,癌组织中E-cadherin相对表达水平降低,Vimentin、β-catenin及TGF-β1 mRNA相对表达水平升高可增加淋巴结转移风险,并可用于预测淋巴结转移。Objective To investigate the correlation between the expression of epithelial mesenchymal transition(EMT)related proteins,transforming growth factor(TGF)-β1 mRNA relative expression levels and lymph node metastasis in tissues of patients with gastric cancer.Methods A total of 51 patients with gastric cancer complicated with lymph node metastasis treated in the hospital from January 2016 to November 2020 were included in the lymph node metastasis group,51 patients with gastric cancer without complicated with lymph node metastasis treated in the hospital at the same period were included in the non lymph node metastasis group.The data of patients were collected and retrospectively analyzed.Regression analysis was used to examine the relationship between tumor differentiation,TNM stage,intravascular tumor thrombus,EMT related proteins(E-cadherin,Vimentin,β-catenin),TGF-β1 mRNA relative expression levels and lymph node metastasis,the predictive efficacy of EMT related proteins and TGF-β1 mRNA relative expression levels for predicting lymph node metastasis in tissues of patients with gastric cancer was analyzed.Results After preliminary comparing the baseline data of the two groups,through regression analysis,the results showed that tumor differentiation,pathological stage,intravascular tumor thrombus condition of patients,E-cadherin,Vimentin,β-catenin and TGF-β1 mRNA relative expression levels in tissues of gastric cancer were related with lymph node metastasis in patients with gastric cancer,low differentiation,TNM stageⅢ-Ⅳ,intravascular tumor thrombus,the decrease of E-cadherin relative expression level and the increase of Vimentin,β-catenin and TGF-β1 mRNA relative expression levels might be risk factors for lymph node metastasis in patients with gastric cancer(OR>1,P<0.05).The receiver operating characteristic curve results showed that the area under the curve of E-cadherin,Vimentin,β-catenin and TGF-β1 mRNA relative expression levels in tissues of gastric cancer for the diagnosis of lymph node metas
