摘  要:目的:基于IKK/IKB/NF-κB信号通路探讨理中汤加味联合穴位埋线干预非酒精性脂肪性肝炎(NASH)大鼠的作用机制。方法:制备NASH大鼠模型,分为模型组、中药+埋线组、埋线组、中药组、西药组,另设空白组。干预4周后,肝组织行光镜观察;生化法检测肝功能、血脂、HOMA-IR,ELISA法检测TNF-α、IL-6、IL-1β含量;Western blot法检测肝脏IKK-α、NF-κB p65、IL-1β、IL-6、TNF-α表达。结果:模型组大鼠肝组织重度脂肪变,肝细胞有较大脂肪细胞广泛浸润,呈“气球状”改变,可见肝小叶内大量炎性细胞浸润,各干预组大鼠肝脏脂肪变性及炎性细胞浸润程度改善,其中以中药+埋线组改善最明显。与模型组比较,各干预组大鼠肝指数、ALT、AST、TG、TC、LDL-C、HOMA-IR、IL-6、IL-1β、TNF-α含量均下降,尤以中药+埋线组降低最明显(P<0.05)。Western Blot检测结果显示,与模型组比较,各干预组大鼠IKK-α、NF-κB p65、IL-1β、IL-6、TNF-α蛋白表达降低,其中中药+埋线组表达最少(P<0.05)。结论:理中汤加味联合穴位埋线可改善NASH大鼠肝功能、血脂水平,调节胰岛素抵抗,促进脂质代谢,改善脂肪变性,其作用机制可能与抑制IKK/IKB/NF-κB信号通路的激活,减少下游炎症细胞因子的释放有关。Objective:To explore the mechanism of Lizhong decoction combined with acupoint catgut embedding in the treatment of non-alcoholic steatohepatitis in(NASH)rats based on IKK/IKB/NF-κB.Methods:The rat model of NASH was made and divided into the model group,traditional Chinese medicine+catgut embedding group,catgut embedding group,traditional Chinese medicine group,and western medicine group.After intragastric administration and catgut embedding intervention for four weeks,the liver tissue was observed under a light microscope,and the liver function,blood lipid,and the contents of TNF-α,IL-6 and IL-1β were detected by HOMA-IR,ELISA method.The expression of IKK-α,NF-κB p65,IL-1β,IL-6 and TNF-αin the liver was detected by Western blot method.Results:In the model group,there was severe steatosis of liver tissue and extensive infiltration of large adipocytes in the liver,showing"balloon"changes,and a large number of inflammatory cell infiltration in the hepatic lobule could be seen.All intervention groups could improve the degree of steatosis and inflammatory cell infiltration in the liver,especially in the traditional Chinese medicine+catgut embedding group.Conclusion:In the model group,the liver index,ALT,AST,TG,TC,LDL-C,HOMA-IR,IL-6,IL-1β,and TNF-αin all intervention groups were decreased,especially in the traditional Chinese medicine+catgut embedding group(P<0.05).The results of Western Blot detection showed that compared with the model group,the protein expressions of IKK-α,NF-κB p65,IL-1β,IL-6 and TNF-αwere decreased in all intervention groups,and the expression in traditional Chinese medicine+catgut embedding group was the least(P<0.05).Lizhong decoction combined with catgut embedding at back Shu acupoint can reduce liver function,blood lipid level,regulate insulin resistance,promote lipid metabolism and improve steatosis,and its mechanism may be related to inhibiting the activation of IKK/IKB/NF-κB signal pathway and reducing the release of downstream inflammatory cytokines.
