不同年龄段晚期肺癌患者尊严水平、生活质量及心理状况的比较分析

摘　　要：目的调查比较不同年龄段晚期肺癌患者尊严水平、生活质量及心理状况的差异,为实施个性化护理提供依据。方法选取2019年9月至2020年7月苏北人民医院收治的156例晚期肺癌患者作为研究对象,按照国际年龄分组标准将其分为中青年组(40~59岁,70例)与老年组(≥60岁,86例),比较两组患者的一般资料、尊严量表(PDI)、生活质量量表(QLQ-C30)、肺癌患者生命质量测定量表(QLQ-L13)、医院用焦虑抑郁量表(HADS)评分。结果有62.79%的老年组患者尊严明显受损,老年组患者的PDI总分、症状困扰、精神安宁、社会支持得分高于中青年组,差异有统计学意义(P<0.05);老年组患者的整体生活质量、情感功能、脱发得分低于中青年组,气促、咯血症状得分高于中青年组,差异有统计学意义(P<0.05);老年组患者的抑郁得分高于中青年组,差异有统计学意义(P<0.05);两组患者的焦虑得分比较,差异无统计学意义(P>0.05)。结论中青年与老年晚期肺癌患者的尊严水平、生活质量及心理状态均有不同程度受损,尤以老年患者更为严重,需要重视针对性采取个性化护理措施,以提高晚期肺癌患者的尊严水平,改善生活质量,避免不良情绪的发生。Objective To investigate and compare the distinctions of dignity,quality of life and psychological status of patients with advanced lung cancer in different age groups,and to provide basis for personalized nursing.Methods A total of 156 patients with advanced lung cancer admitted to Subei People′s Hospital from September 2019 to July 2020 were selected as the research subjects.According to the international age grouping standard,they were divided into young and middle-aged group(40-59 years old,70 cases)and old group(≥60 years old,86 cases).General information,patient dignity inventory(PDI),quality of life questionnaire(QLQ-C30),lung cancer patient quality of life questionnaire(QLQ-L13)and hospital anxiety and depression scale(HADS)scores were compared between the two groups.Results The dignity of 62.79%of the elderly group was significantly impaired,and the scores of PDI,symptom distress,mental tranquility and social support in the elderly group were higher than those in the young and middle-aged group,and the differences were statistically significant(P<0.05).The scores of overall quality of life,emotional function and hair loss in the elderly group were lower than those in the young and middle-aged group,while the scores of shortness of breath and hemoptysis were higher than those in the young and middle-aged group,and the differences were statistically significant(P<0.05).The depression score of the elderly group was higher than that of the young and middle-aged group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in anxiety scores between the two groups(P>0.05).Conclusion The dignity level,quality of life and psychological state of middle-aged,young and elderly patients with advanced lung cancer are impaired to varying degrees,especially the elderly patients.It is necessary to pay attention to targeted personalized nursing measures to improve the dignity level of patients with advanced lung cancer,improve the quality of life,and avoid the occurr
