中医溻渍联合房中术三采嬉戏法治疗原发性早泄的多中心临床观察

摘　　要：目的观察中医溻渍法联合房中术三采嬉戏法治疗早泄的有效性。方法采用随机分组,早泄患者108例随机分组,分为治疗组72例,每天2次,每次15min,将五香花外洗方(五倍子、公丁香等)使用中医溻渍法配合揉搓作用于阴茎及阴茎头冠状沟表面;对照组36例,性生活前15min,将利多卡因液涂于阴茎头、系带、冠状沟部;两组均配合采用传统房中术三采嬉戏法作为基础治疗。治疗期间每周至少1次性生活,连续8周。记录评估治疗前、后及结束后4周的早泄诊断工具(PEDT)、中国早泄功能评分表(CIPE-Q6+Q7)、阴道内射精潜伏时间(IELT)、阴茎敏感性(SW振动值)等指标,并随访不良反应。结果治疗组和对照组总有效率分别为77.8%和50.0%;组间临床疗效比较,差异有统计学意义(P<0.05)。组内比较,治疗组PEDT,CIPE-Q6+Q7、IELT,以及SW振动值均有改善,且治疗结束后4周虽疗效有所下降(P<0.05),但仍优于治疗前(P<0.05);对照组PEDT,CIPE-Q6+Q7、IELT有所改善,但治疗结束后4周疗效大都回复至治疗前(P>0.05);治疗组各个量表治疗结束及结束后4周的指标均优于对照组,差异均有统计学意义(P<0.05),且对照组出现的勃起下降等不良反应没有在治疗组发生。结论按疗程使用中医溻渍法联合房中术三采嬉戏法治疗早泄安全,且疗效显著,可以改善患者的早泄临床症状,延长阴道内射精潜伏时间,缓解阴茎敏感性,提高性生活配偶双方满意度,不良反应低,疗效可以维持,为早泄的临床治疗提供中医方法的实践经验,适合临床推广。Objective To observe the efficacy of"Tazi"method combined with the Sancaixixi therapy in TCM treatment of premature ejaculation.Methods Total of 108 patients with premature ejaculation were randomly divided into the treatment group(72 cases)and the control group(36 cases).In the treatment group,the Chinese herbal medicine external washing formula"Wuxianghuafang"(gallnut,clove,etc.)was used to stained on the surface of the penis of the patients by the"Tazi"therapy of TCM-the specific operation:Wet gauze or cotton wool containing Chinese herbal medicine on the penis head,ligament and coronal groove for 10 min,then soak the above part in the Chinese medicine liquid,and continue to rinse the above part with the Chinese medicine liquid for 5 min.In the control group,lidocaine solution was applied to the penis head,ligament,and coronal sulcus of the patients.All the patients received the traditional"Sancaixixi"therapy as the basic treatment.followed up for 8 weeks and 4 weaks after the discontinuation of treatment,PEDT,CIPE-Q6+Q7,IELT and SW index were recorded and evaluated.Adverse reactions were observed.Results The total effective rates of the treatment group and the control group were 77.8%and 50.0%respectively,and the difference was statistically significant(P<0.05).In the treatment group,PEDT,CIPE-Q6+Q7,IELT,and SW vibration values were all improved.The curative effect was decreased4 weeks after treatment(P<0.05),but it was still better than that before treatment(P<0.05).In the control group,PEDT,cipe-q6+Q7,IELT were all improved,but the curative effect at 4 weeks after treatment mostly returned to that before treatment(P>0.05);The indexes of each scale in the treatment group were better than those in the control group at the end of treatment and 4 weeks after the end of treatment,and the differences were statistically significant(P<0.05),and the adverse reactions such as erectile decline in the control group did not occur in the treatment group.Conclusion"Tazi"method combined with the"Sancaixixi"therapy can amel
