消化道神经内分泌肿瘤病理分级的危险因素分析

摘　　要：目的分析影响消化道神经内分泌肿瘤(GI-NENs)病理分级的危险因素。方法绘制ROC曲线分析74例GI-NENs患者年龄、中性粒细胞/淋巴细胞比值(NLR)和血白蛋白(Alb)水平对病理分级的预测效能,分别行单因素logistic回归模型和多因素logistic回归模型分析影响GI-NENs病理分级的危险因素。结果年龄58岁、NLR 1.68和Alb 42.9 g/L为最佳截断值,ROC曲线下面积分别为0.835、0.661和0.830。年龄>58岁、原发部位为上消化道、NLR>1.68和Alb≤42.9 g/L是影响GI-NENs病理分级的危险因素(P<0.05)。结论年龄、肿瘤原发部位和治疗前NLR和Alb水平是GI-NENs病理分级的影响因素。Objective To analyze the risk factors for the pathological grading of gastrointestinal neuroendocrine neoplasms(GI-NENs).Methods The ROC curve was drawn to analyze the predictive efficiency of age, neutrophil-lymphocyte ratio(NLR) and blood albumin(Alb) level on pathological grade of 74 patients with GI-NENs.Univariate logistic regression model and multivariate logistic regression model were used to analyze the risk factors affecting the pathological grade of GI-NENs.Results Taking fifty-eight years old, NLR 1.68 and Alb 42.9 g/L as the best cut-off values, the areas under ROC curve were 0.835,0.661 and 0.830,respectively.Age older than 58 years, primary site of upper digestive tract, NLR bigger than 1.68 and Alb no more than 42.9 g/L were the risk factors affecting the pathological grade of GI-NENs(P<0.05). Conclusion Age, primary tumor site, levels of NLR and Alb before treatment are the factors affecting the pathological grade of GI-NENs.
