原发性肠脂垂炎的CT表现及发病特征相关因素分析

摘　　要：目的分析原发性肠脂垂炎(PEA)的影像学表现及发病特征相关因素。方法回顾性分析经手术或随访证实的43例PEA患者的CT表现,并对患者年龄、性别、发病部位及腹腔内脏脂肪面积(VFA)等相关特征进行分析。结果43例患者中4例为多发,共47个病灶,6例为陈旧钙化病灶,其余均表现为结肠旁类圆形、卵圆形或类梭形脂肪密度肿块,部分病变内见云絮状、斑片状稍高密度影,边缘稍高密度环影,部分病灶周围可见渗出及邻近腹膜增厚。47个病灶中33个病灶位于降结肠区(70.2%);10个病灶位于乙状结肠区(21.3%);3个病灶位于升结肠区(6.4%);1个病灶位于盲肠区(2.1%)。43例患者腹腔VFA分布范围69.24~314.5 cm^(2);以VFA=100 cm^(2)为界值,10例患者VFA小于此界值(23.3%);33例患者VFA大于此界值(76.7%)。结论PEA具有一定的特征性影像学表现,结肠旁的类圆形、卵圆形脂肪密度肿块,周围可见环形征,部分可见中心点征,周围伴或不伴炎性渗出及腹膜增厚。Objective To analyze the imaging features and related factors of primary epiploic appendagitis(PE A).Methods The CT features of 43 patients with PEA confirmed by operation or follow-up were analyzed retrospectively.The age and sex of the patients,the location of lesions,and the abdominal visceral fat area(VFA)were analyzed.Results Among the 43 patients,4 patients had multiple lesions and there were 47 lesions in total.Except of 6 patients with calcification lesions,the other patients had lesions showing round,ovoid or fusiform fat density masses adjacent to the colon.In some lesions,there were cottony,patchy slightly high-density in the center of the lesions,rim slightly high-density on the edge of the lesions.effusion and peritoneal thickening around the lesions.Of the 47 lesions,33 around the descending colon(70.2%),10 around the sigmoid colon(21.3%),3 around the ascending colon(6.4%),and 1 around the cecum(2.1%).The distribution range of VFA was 69.24-314.5 cm^(2).As the cutoff of 100 cm^(2),the VFA was less in 10 patients(23.3%),and greater in 33 patients(76.7%).Conclusion PEA has characteristic CT manifestations,which is the round or oval fat density mass beside the colon,surrounded by a hyperdense ring,the central dot sign,with or without inflammatory effusion or peritoneal thickening.
