摘　　要：[目的]探讨胃癌血清Glypican-3(serum Glypican-3,sGPC3)及胃癌组织GPC3(tissue Glypican-3,tGPC3)的表达水平及其与临床病理因素的相关性和预后价值。[方法]应用IHC检测139例胃癌组织tGPC3的表达,应用ELISA测定胃癌患者术前sGPC3的水平,分析其与胃癌患者临床病理因素的相关性。选择101例健康体检者血清标本检测sGPC3水平作对照。[结果]胃癌tGPC3的总表达率为10.07%(14/139),其中局灶阳性为5.75%(8/139),弥漫阳性为4.32%(6/139);胃癌sGPC3的水平为(3.55±0.45)ng/ml,与正常健康人群无明显差异;而在组织水平局灶或弥漫表达GPC3的胃癌患者中,其sGPC3水平组间比较差异有统计学意义(P<0.05);高水平的sGPC3与tGPC3、淋巴血管侵犯、pT分期、淋巴结转移、远处转移及pTNM分期显著相关(P<0.05);单因素分析结果显示sGPC3高表达与胃癌患者的不良预后有相关性(P<0.05),但多因素分析未显示sGPC3是影响患者预后的独立危险因素(P>0.05)。Kaplan-Meier生存分析显示,胃癌患者的总生存时间随着sGPC3的表达增强显著下降(P<0.05)。[结论]sGPC3高水平与胃癌患者的预后密切相关,术前筛查sGPC3可能会为筛选GPC3相关的免疫治疗的胃癌患者提供帮助。[Objective]To investigate the tissue Glypican-3(tGPC3)expression in gastric cancer and the predicting value of serum Glypican-3(sGPC3)in gastric cancer prognosis.[Methods]We detect the level of sGPC3 by ELISA in 139 cases of gastric carcinoma and 101 cases of healthy subjects.The tGPC3 from the gastric carcinoma patients was also detected by using immunohistochemical staining.Correlations with clinicopathological variables were analyzed respectively.[Results]The positive rate of tGPC3 in specimens of gastric carcinoma was 10.07%(14/139),in which focal and diffuse positive rates were 5.75%(8/139)and 4.32%(6/139)respectively.The average level of sGPC3 in gastric cancer patients was 3.55±0.45 ng/ml,which was not significantly different from the healthy subjects(P>0.05).However,the sGPC3 was significantly higher in the gastric carcinoma patients with tGPC3 focally or diffusely expression(P<0.05).sGPC3 level was positively associated with tGPC3,lymphovascular invasion,pT stage,lymph node metastasis,distant metastasis,and pTNM(P<0.05).Univariate analysis showed that a high level of sGPC3,tumor size,lymphovascular invasion,perineural invasion,pT stage,pN stage,and pTNM stage were prognostic factors for poorer overall survival of gastric cancer patients(P<0.05).However,multivariate analysis showed sGPC3 wasn’t an independent risk factor for poor prognosis of gastric cancer(P>0.05).Kaplan-Meier survival analysis showed that the overall survival rate of gastric cancer patients decreased significantly with the increase of sGPC3 level(P<0.05).[Conclusion]The sGPC3 level is a significant marker for gastric carcinoma patients.The evaluation of sGPC3 may become a potential biomarker to identify the precise patient for GPC3 immunotherapy in gastric cancer.
