抑郁症严重程度与认知功能及事件相关电位的相关性分析

摘　　要：目的探讨抑郁症严重程度与患者认知功能及事件相关电位(ERP-P300)的相关性。方法选择本院2018年2月~2020年2月期间收治的84例抑郁症患者作为研究对象,根据汉密尔顿抑郁量表(HAMD)评分分为重度组(28例)和轻中度组(56例);同时选择一般资料相匹配的40例健康人群设为对照组。采用重复性成套神经心理状态测验(RBANS)评估患者认知功能,采用Viking Quest 4通道肌电诱发电位仪评估患者ERP-P300,采用Pearson相关分析法分析各指标间的相关性。结果(1)重度和轻中度组的HAMD评分高于对照组(P<0.05);重度和轻中度组的RBANS各项评分(即刻记忆、视觉广度、语言功能、注意、延时记忆评分)和总分低于对照组,且重度组的评分低于轻中度组(P<0.05)。(2)重度和轻中度组P3a、P3b、N2的潜伏期高于对照组,且重度组的潜伏期高于轻中度组(P<0.05);重度和轻中度组P3a、P3b、N2的波幅低于对照组,且重度组的波幅低于轻中度组(P<0.05)。(3)HAMD评分与RBANS评分呈显著负相关;HAMD评分与P3a、P3b、N2波幅呈负相关,与P3a、P3b、N2潜伏期呈正相关,RBANS评分与P3a、P3b、N2波幅呈正相关,与P3a、P3b、N2潜伏期呈负相关(P<0.05)。结论抑郁症患者均存在不同程度的认知功能损害,且抑郁程度越高其认知功能损害越严重,ERP-P300可反映抑郁症患者认知功能的损害程度。Objective To explore the correlation between the severity of depression and cognitive function and event-related potentials(ERP-P300).Methods A total of 84 patients with depression admitted to our hospital from February 2018 to February 2020 were selected.According to the Hamilton Depression Scale(HAMD)score,the patients were divided into severe group(28 cases)and mild and moderate group(56 cases).At the same time,40 healthy people with matched general data were selected as the control group.The repeated neuropsychological state test(RBANS)was used to evaluate the cognitive function of the patients,the Viking Quest 4-channel potentiometer was used to evaluate the ERP-P300 of the patients,and Pearson correlation analysis was used to analyze the correlation between the indicators.Results(1)The HAMD score in the severe and mild and moderate groups was higher than that in the control group(P<0.05).The RBANS(immediate memory,visual breadth,language function,attention,and delayed memory)and total scores in severe and mild and moderate groups were significantly lower than those in the control group,while the severe group had lower scores than the mild and moderate group(P<0.05).(2)The incubation period of P3 a,P3 b and N2 in the severe and mild and moderate groups was higher than that in the control group,and the incubation period of the severe group was higher than that in the mild and moderate group(P<0.05).The amplitude of P3 a,P3 b and N2 in the severe and mild and moderate groups was lower than that in the control group,and the amplitude of P3 a,P3 b and N2 in the severe and mild and moderate groups was lower than that in the mild and moderate groups(P<0.05).(3)There was a significant negative correlation between HAMD score and RBANS score.The HAMD score was negatively correlated with the amplitude of P3 a,P3 b,N2,and positively correlated with the incubation of P3 a,P3 b,and N2,while the RBANS were positively correlated with the amplitude of P3 a,P3 b,N2,and negatively correlated with the incubation of P3 a,P3 b(P
