“从阳引阴”为主、“从阴引阳”为辅探析艾灸审穴论治帕金森病肝肾阴虚证

摘　　要：艾灸配合中药、针灸可明显改善帕金森病患者的病情,但以肝肾阴虚为核心病机的帕金森病是否可灸尚未形成统一认识。从艾灸的方式、"从阳引阴,从阴引阳"和艾灸补泻法审穴论治3个方面阐述帕金森病肝肾阴虚证可灸的理由。并在"阴阳互根"的理论指导下,以核心病机为出发点,提出以"从阳引阴"为主、"从阴引阳"为辅的艾灸治则。"从阳引阴"包括补法灸百会、四神聪以"固阳益阴精",泻法灸大椎、风门、四花穴、手足三里以泻虚火、清痰热和祛肝风;"从阴引阳"则包括补法灸涌泉、神阙、关元、气海以助阳化阴。若患者在肝肾阴虚的基础上兼夹气虚、血瘀等证,则可加灸中脘、下脘、心俞、血海。Moxibustion combined with traditional Chinese medicine or acupuncture can significantly improve the condition of patients with Parkinson’s disease.However,whether moxibustion can be used in Parkinson’s disease with liver-kidney Yin deficiency as the core pathogenesis has not yet formed a unified understanding.This paper expounds the reasons for moxibustion of liver-kidney Yin deficiency syndrome of Parkinson’s disease from three aspects:the way of moxibustion,"inducing Yin from Yang,inducing Yang from Yin"and replenishing and reducing methods of moxibustion.Moreover,under the guidance of the theory of"mutual root of Yin and Yang",taking the core pathogenesis as the starting point,the moxibustion treatment principle with"inducing Yin from Yang"as the main and"inducing Yang from Yin"as the supplement was put forward."Inducing Yin from Yang"includes moxibustion at Baihui(DU20),Sishencong(EX-HN1)for replenishing method to"consolidate Yang and benefit Yin essence",moxibustion at Dazhui(DU14),Fengmen(BL12),Sihua acupoints,and Shousanli(LI10)and Zusanli(ST36)for reducing method to relieve deficiency fire,clear phlegm heat and expel liver wind."Inducing Yang from Yin"includes moxibustion at Yongquan(KI1),Shenque(RN8),Guanyuan(RN4),and Qihai(RN6)for replenishing method to help Yang and transform Yin.If the patient has Qi deficiency,blood stasis and other syndromes on the basis of liver-kidney Yin deficiency,moxibustion can be added to Zhongwan(RN12),Xiawan(RN10),Xinshu(BL15)and Xuehai(SP10).
