摘　　要：目的评估舒芬太尼超前镇痛对行全腮腺切除术的老年患者术后疼痛的效果及安全性。方法选取2019年1月~2020年6月全麻下行全腮腺切除术治疗的72例老年患者,按照随机数表法分为试验组和对照组,各36例。试验组于麻醉诱导前10 min静脉注射0.15μg/kg舒芬太尼超前镇痛,对照组注射等量生理盐水。应用视觉模拟评分法(VAS)对两组患者术后1、2、4、8、12、24 h进行疼痛评价、记录术后不良反应及追加镇痛药物情况。结果试验组患者术后1、2、4 h的VAS评分低于对照组,术后需要追加镇痛药物比例低于对照组,术后恶心、呕吐、呼吸抑制的总发生率低于对照组,差异均有统计学意义(P<0.05)。结论舒芬太尼超前镇痛可有效减轻老年全腮腺切除术患者术后的急性疼痛,减少术后镇痛药物的追加和恶心、呕吐、呼吸抑制等不良反应的发生,提高麻醉满意度。Objective To evaluate the efficacy and safety of preemptive analgesia with sufentanil on postoperative pain in older patients undergoing total parotidectomy.Methods A total of 72 older patients undergoing total parotidectomy in Hospital of Stomatology of Jilin University from January 2019 to June 2020 were selected.The patients were randomly divided into the experimental group and the control group, 36 cases in each group.Patients in the experimental group received an intravenous injection of 0.15 μg/kg sufentanil before the induction of anesthesia, while patients in the control group received the same volume of saline.Postoperative analgesia was assessed by using visual analogue scale(VAS) at 1,2,4,8,12 and 24 h after surgery.The incidence of adverse reactions and additional analgesia consumption were recorded.Results In the experimental group, VAS scores at 1,2,4 h after the surgery were significant lower, the proportion of patients requiring additional analgesia was lower, and the incidences of adverse reactions were significantly lower than those in the control group, there were significant differences between the groups(P<0.05).Conclusion Preemptive analgesia with sufentanil might release the postoperative pain in older patients undergoing total parotidectomy, reduce the additional analgesia consumption and the incidences of adverse reactions.
