^(32)P持续低剂量辐射敷贴治疗表浅性婴幼儿皮肤血管瘤疗效观察

摘　　要：目的观察磷-32(^(32)P)持续低剂量放射性敷帖治疗婴幼儿表浅性皮肤血管瘤的疗效。方法回顾性分析陕西省肿瘤医院核医学科2018年1月至2020年1月采用^(32)P持续低剂量敷帖治疗的576例表浅型婴幼儿皮肤血管瘤患者的临床资料,通过门诊复诊、电子病历或电话随访,评价^(32)P敷贴法的治疗效果及不良反应。结果576例患儿疗效评估Ⅰ级26例(4.5%),Ⅱ级81例(14.1%),Ⅲ级159例(27.6%),Ⅳ级310例(53.8%),显效率为81.4%;不同年龄、不同性别及不同病灶位置的治疗效果比较差异均无统计学意义(P>0.05);不同大小病灶的治疗效果比较差异有显著统计学意义(P<0.01);治疗期间所有患儿均无明显全身不良反应,仅15例出现照射部位脱屑,2例出现患处局部皮肤轻度水肿,无需特殊处理;1例出现湿性皮炎,中止敷贴治疗。结论^(32)P敷贴法治疗表浅型婴幼儿皮肤血管瘤疗效肯定,且具有无痛、安全、简便易行、副作用轻的优点,易被患儿及家属接受,可于临床上推广应用。Objective To observe the therapeutic effect of low-dose-rate radiation of radionuclide phosphorus-32 (^(32)P) in the treatment of superficial infantile hemangiomas.Methods The clinical data of 576 patients of superficial infantile hemangiomas treated with low-dose-rate radiation of radionuclide^(32)P from January 2018 to January 2020 in Department of Nuclear Medicine,Shaanxi Provincial Cancer Hospital were retrospectively analyzed.The therapeutic and side effect of^(32)P were evaluated and analyzed through out-patient visit,electronic medical record or telephone follow-up.Results A total of 576 patients were included in this study,of whom 26 cases (4.5%) were rated as gradeⅠ,81 cases (14.1%) as gradeⅡ,159 cases (27.6%) as gradeⅢ,and 310 cases (53.8%) as gradeⅣ.The overall therapeutic efficacy was 81.4%.Different ages,gender and lesion location showed no statistically significant difference in the therapeutic effect (all P>0.05).There was statistically significant difference in the treatment effect of different size lesions (P<0.001).There was no significant systemic adverse reaction in 576 cases during treatment,of whom 15 cases showed desquamation at the irradiation site,and 2 cases showed mild skin edema at the affected site (without any special treatment).Wet dermatitis occurred in only 1 case,and the treatment was discontinued.Conclusion Low-dose-rate radiation of radionuclide^(32)P has certain efficacy for superficial infantile hemangioma.Furthermore,it can be easily accepted by infants and young children and their families with the advantages of painless,safe,simple,easy to perform,and light side effects.Thus,it should be popularized in clinical application.
