宫颈癌VMAT外照射联合三维腔内后装治疗的近期疗效和不良反应分析

摘　　要：目的分析宫颈癌VMAT外照射联合三维腔内后装治疗的近期疗效和不良反应。方法选取2019年12月至2020年4月本院收治的70例宫颈癌患者作为研究对象,随机数字表法分为两组,各35例。对照组患者采用VMAT外照射联合二维腔内后装治疗,观察组患者采用VMAT外照射联合三维腔内后装治疗。比较两组近期疗效、放化疗相关指标、不良反应发生率、局部复发率与远处转移率。结果观察组临床好转率高于对照组,不良反应发生率低于对照组(P<0.05);观察组局部复发率及远处转移率均明显低于对照组(P<0.05);观察组GTVnd、CTV、PTV及PGTVnd均明显高于对照组(P<0.05)。结论 VMAT外照射联合三维腔内后装治疗宫颈癌的近期疗效显著,且安全性较高,值得临床推广应用。Objective To analyze the short-term efficacy and adverse reactions of VMAT external beam irradiation combined with three-dimensional intracavitary afterloading treatment for cervical cancer. Methods A total of 70 patients with cervical cancer admitted to our hospital from December 2019 to April 2020 were selected as the research subjects. They were divided into two groups according to the random number table method,with 35 cases in each group. Patients in the control group were treated with VMAT external beam irradiation combined with two-dimensional intracavity afterloading treatment, and patients in the observation group were treated with VMAT external beam irradiation combined with three-dimensional intracavitary afterloading treatment. The short-term efficacy, radio-chemotherapy-related indicators and adverse reactions, the local recurrence rate and distant metastasis rate were compared between the two groups. Results The clinical improvement rate of the observation group was higher than that of the control group, and the incidence of adverse reactions was lower than that of the control group(P<0.05);the local recurrence rate and distant metastasis rate of the observation group were significantly lower than those of the control group(P<0.05);the GTVnd, CTV, PTV and PGTVnd in the observation group were significantly higher than the control group(P<0.05). Conclusion VMAT external beam irradiation combined with three-dimensional intracavitary afterloading has significant short-term curative effect and high safety in the treatment of cervical cancer. It is worthy of clinical application.
