双链法与金属直丝弓托槽矫治器联合口外弓支抗技术对正畸牙牙间隙关闭的疗效比较

摘　　要：目的:探究金属直丝弓托槽矫治器和双链法联合口外弓支抗技术在改善正畸牙牙间隙关闭的效果。方法:随机将上下颌的牙列间隙不同正畸患者82例分为两组,研究组41例患者均实施双链法治疗,对照组41例患者均实施口外弓支抗技术结合金属的直丝弓进行托槽矫治器治疗。分别记录上颌尖牙、第一、二前磨牙、第一磨牙的情况。结果:双链法治疗的平均时间为12.3±0.8个月,口外弓支抗技术结合金属的直丝弓进行托槽矫治器的平均时间为17.1±1.4个月,差异有统计学意义(P<0.05);两组患者X线头影测量结果差异有统计学意义(P<0.05),具有统计学意义。和矫治前对比,两组患者矫治半年后的SPD、GI、SBI以及PLI指标均改善明显(P<0.05),但两组间SPD、GI、SBI差异无统计学意义(P>0.05);矫治后半年双链法的PLI的指标显著优于金属直丝弓托槽矫治器联合口外弓支抗技术组(P<0.05)。结论:双链法关闭拔牙间隙,能够缩短拔牙间隙关闭时间,且在上颌第一、二前磨牙、第一磨牙间宽度改善方面,矫治效果更好。Objective:To investigate the efficacy of double⁃chain method vs metal straight⁃wire arch bracket appliance combined with external arch anchorage in improving the interdental space closure of orthodontic teeth.Methods:Eightly⁃two orthodontic patients with inconsistent gaps of the maxillary and mandibular dentitions were randomly divided into two groups.The 41 patients in the study group were treated with double⁃chain method,and other 41 patients in the control group were treated with external arch anchorage combined with metal straight⁃wire arch bracket appliance.The conditions of the maxillary canines,first and second premolars,and the first molar were recorded.Results:The mean treatment time with double⁃chain method was 12.3±0.8 months vs 17.1±1.4 months with extranal arch anchorage combined with metal straight wire arch bracket appliance,with significant difference between groups(P<0.05).The X⁃ray cephalometrics differed significantly between the two groups of patients(P<0.05).SPD,GI,SBI and PLI were improved significantly at half a year of treatment compared with baseline(P<0.05)in either group,but there was no difference in SPD,GI and SBI between the two groups(P>0.05).The value of PLI was significantly more favorable with double⁃chain method than with metal straight wire arch bracket appliance combined with external arch anchorage(P<0.05).Conclusion:The double⁃chain method can shorten the time to closure of the extraction gap,with better orthodontic outcome in improving the interdental space between the maxillary first and second premolars and the first molar.
