喹硫平联合丙戊酸镁治疗双相情感障碍的临床效果及对患者炎性因子、认知功能的影响

摘　　要：目的探讨喹硫平联合丙戊酸镁治疗双相情感障碍的临床效果及对患者炎性因子、认知功能的影响。方法选取2019年2月—2020年10月本院收治的双相情感障碍102例,根据治疗方式的不同分为研究组53例和对照组49例,研究组给予喹硫平联合丙戊酸镁治疗,对照组仅予丙戊酸镁治疗,2组均连续治疗1个月。比较2组临床疗效,治疗前后Bech-Rafaelsen躁狂量表(BRMS)评分,治疗前后炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)和白细胞介素-4(IL-4)],治疗前后认知功能[Wisconsin卡片分类测验、持续操作测验(CPT)、Hopkins词语学习测验修订版(HVLT-R)评分],以及不良反应发生情况。结果治疗后研究组总有效率高于对照组(P<0.05)。治疗后2组BRMS评分及TNF-α、IL-1β、IL-4水平均明显降低,其中以研究组降低最为显著(P<0.05,P<0.01)。治疗后2组CPT、HVLT-R评分均明显升高,其中以研究组CPT评分升高最为显著(P<0.05,P<0.01)。2组不良反应总发生率比较差异无统计学意义(P>0.05)。结论喹硫平联合丙戊酸镁治疗可有效提高双相情感障碍患者临床效果,改善患者临床症状及炎性因子水平,提高认知功能,且安全可靠。Objective To investigate clinical efficacy of Quetiapine combined with Magnesium Valproate in treatment of patients with bipolar disorder and its effects on inflammatory factors and cognitive function.Methods A total of 102 patients with bipolar disorder admitted between February 2019 and October 2020 were selected and divided into research group(n=53)and control group(n=49)according to different therapeutic methods.Research group was treated with Quetiapine combined with Magnesium Valproate,while control group was treated with Magnesium Valproate only.Both groups were treated for 1 month continuously.Clinical efficacy,Bech-Rafaelsen manic rating scale(BRMS)scores and inflammatory factors[tumor necrosis factor-α(TNF-α),Interleukin-1β(IL-1β)and interleukin-4(IL-4)],cognitive functions[Wisconsin Card Sorting Test,continuous operation test(CPT),Hopkins Verbal Learning Test revised version(HVLT-R)score]before and after treatment as well as adverse reactions were compared between two groups.Results The total effective rate in research group was significantly higher than that in control group(P<0.05).After treatment,BRMS scores and levels of TNF-α,IL-1βand IL-4 were significantly decreased in two groups,and the decreased degrees were more obvious in research group(P<0.05,P<0.01).After treatment,CPT and HVLT-R scores were significantly increased in two groups,and the increased CPT score in research group was the most significant(P<0.05,P<0.01).There was no significant difference in the total incidence rate of adverse reactions between two groups(P>0.05)Conclusion Quetiapine combined with Magnesium Valproate may effectively improve clinical efficacy,improve clinical symptoms and levels of inflammatory factors and enhance cognitive function with safe and reliable in treatment of patients with bipolar disorder.
