摘 要:目的分析MBT直丝弓矫治器+微型种植体支抗(MIA)技术治疗安氏Ⅱ~1分类错[牙合]畸形的效果。方法选取我院86例安氏Ⅱ~1分类错[牙合]畸形患者(2017年4月至2019年4月),随机数字表法分组。HGA组43例给予MBT直丝弓矫治器+口外弓支抗(HGA)治疗,MIA组43例以MBT直丝弓矫治器+MIA技术治疗。比较两组美观效果、拔牙间隙关闭时间、矫治时间、磨牙位移及矫正前后牙齿咬合力、咀嚼功能、龈沟液一氧化氮(NO)、细胞因子细胞间黏附分子1(CAM1)、基质金属蛋白酶-2(MMP-2)含量。结果MIA组矫正效果优于HGA组,拔牙间隙关闭时间、矫治时间短于HGA组,磨牙位移小于HGA组(P<0.05);与HGA组比较,矫正后MIA组牙齿咬合力、咀嚼功能较高,龈沟液NO、CAM1、MMP-2水平较低(P<0.05)。结论MBT直丝弓矫治器+MIA技术能有效缩短安氏Ⅱ~1分类错[牙合]畸形治疗周期,下调龈沟液NO、CAM1、MMP-2水平,改善牙齿咬合力及咀嚼功能,提升美观效果。Objective To analyze the effect of MBT straight wire appliance+mini-implant anchorage(MIA)technology in the treatment of Class II-1 malocclusion.Methods 86 patients with malformation of Angle’sⅡ~1 classification in our hospital(April 2017-April 2019)were selected and grouped by random number table.Forty-three patients in the HGA group were treated with MBT straight wire appliance+external arch anchorage(HGA),and 43 patients in the MIA group were treated with MBT straight wire appliance+MIA technique.Compare the aesthetic effects of the two groups,the closing time of the extraction gap,the correction time,the displacement of the molars and the occlusal force of the teeth before and after the correction,mastication function,nitric oxide(NO)in the gingival crevicular fluid,cytokine intercellular adhesion molecule 1(CAM1),matrix metalloproteinases-2(MMP-2)content.Results The correction effect of the MIA group was better than that of the HGA group.The closing time of the extraction gap and the correction time were shorter than those of the HGA group,and the molar displacement was smaller than that of the HGA group(P<0.05).Compared with the HGA group,the MIA group had higher teeth occlusal force and chewing function after correction,Gingival crevicular fluid NO,CAM1 and MMP-2 levels were low(P<0.05).Conclusion The MBT straight wire appliance+MIA technology can effectively shorten the treatment cycle of Class II~1 malocclusion,lower the levels of gingival crevicular fluid NO,CAM1,and MMP-2,improve the occlusal force and chewing function of teeth,and enhance the aesthetic effect.
