穴位贴敷改善大肠息肉切除术后腹部不适症状的临床疗效观察

摘　　要：目的:观察中药穴位贴敷(神阙穴,升阳益气贴)在改善大肠息肉患者术后腹部不适症状(腹胀、腹痛、腹泻)的临床效果。方法:选择2019年8月—2021年9月在中国中医科学院望京医院住院接受结肠镜下息肉切除治疗后出现腹部不适症状的180例患者,根据随机数字表法分为对照组和试验组,每组90例。对照组给予电蜡疗,试验组在电蜡疗治疗基础上联合穴位贴敷治疗。记录术后72 h内腹部不适症状的情况,并进行打分。结果:术后1 h试验组和对照组腹胀、腹痛症状积分比较差异无统计学意义,术后24 h、48 h试验组腹痛VAS评分显著低于对照组(P<0.01);术后24 h试验组腹胀症状评分显著低于对照组(P<0.01);术后72 h试验组患者腹泻率明显低于对照组(P<0.05)。试验组中有2例患者脐周皮肤轻微发红,无一例出现瘙痒、水疱等情况。结论:升阳益气贴贴于神阙穴有助于改善大肠息肉术后患者腹痛、腹泻症状,可降低术后腹泻发生率,并且不良反应轻微,是一种安全有效的经典中医外治疗法。Objective:To observe the clinical effect of Chinese medicine point application(Shenque point,Shengyang Yiqi plaster)in improving abdominal discomfort(abdominal distension,abdominal pain,diarrhea)of patients with colorectal polyps after operation.Methods:A total of 180 inpatients who developed abdominal discomfort after receiving colonoscopic polypectomy in Wangjing Hospital,Chinese Academy of Chinese Medical Sciences from August 2019 to September 2021 were included.They were divided into the control group and the experimental group according to the random number table,with 90 cases in each group.The control group was given electric wax therapy,and the experimental group was given combined acupoint application therapy on the basis of electric wax therapy.Abdominal discomfort within 72 h after surgery was recorded and scored.Results:There was no significant difference in the scores of abdominal distension and pain within 1 h after operation between the two groups.The VAS scores of abdominal pain within 24 h and 48 h after operation in the experimental group were significantly lower than those in the control group(P<0.01).The abdominal distension symptom score of the experimental group 24 h after surgery was significantly lower than that of the control group(P<0.01).The diarrhea rate in the experimental group was significantly lower than that in the control group within 72 h after operation(P<0.05).Two patients in the experimental group had slight redness of the skin around the navel,and none of them had pruritus or blisters.Conclusion:Shengyang Yiqi plaster applied to Shenque is a safe and effective classical external therapy in traditional Chinese medicine,and it can help to improve the symptoms of abdominal pain and diarrhea in patients after colorectal polypectomy,reduce the incidence of postoperative diarrhea,and has mild adverse events.
