血清外泌体中B7-H4在黑素瘤中的表达及其临床意义

摘　　要：目的:探讨黑素瘤患者血清外泌体中B7-H4的表达及其临床意义。方法:采用超速离心法提取患者血清内的外泌体,采用透射电镜观察外泌体超微结构,并用Western Blot法检测外泌体中B7-H4蛋白的表达。利用酶联免疫吸附试验(ELISA)检测60例黑素瘤患者和60例健康体检者血清来源的外泌体内B7-H4的表达水平,并探讨临床病理指标和预后的关系。结果:黑素瘤患者血清来源的外泌体中B7-H4的表达水平较健康对照组明显升高,差异有统计学意义(P<0.05)。血清外泌体中B7-H4高表达组中Breslow厚度>4 mm、有溃疡、Ⅲ~Ⅳ期及核分裂象≥5个/mm;患者例数显著高于低表达组(P<0.05)。血清外泌体中B7-H4高表达组患者生存时间较低表达组短,差异有统计学意义(P<0.05)。调整了混杂因素后,多因素分析显示血清外泌体中B7-H4表达是黑素瘤患者的独立预后因子(P<0.05)。结论:血清外泌体B7-H4表达升高与黑素瘤的预后不良有关。测定血清外泌体内B7-H4的水平可能有助于黑素瘤的临床预后判断。Objective:To investigate the expression and clinical significance of B7-H4 in the serum exosomes of patients with malignant melanoma. Methods:Exosomes were extracted from patients’ serum by overspeed centrifugation and proved by transmission electron microscope(TEM), and the expression of B7-H4 protein in exosomes was detected by Western Blot. Then, ELISA was used to detect the expression level of B7-H4 in exosomes from the serum of 60 melanoma patients and 60 healthy donors. The relation ship between exosomal B7-H4 expression and clinical pathological parameters and prognosis in malignant melanoma patients were analyzed. Results:The expression level of exosomal B7-H4 in malignant melanoma patients was significantly higher than that in healthy subjects(P<0.05). The number of cases with Breslow thickness >4 mm, ulcer, stage Ⅲ-Ⅳ and mitotic image≥5/mm;in the high B7-H4 expression group was significantly higher than that in the low expression group(P<0.05). In addition, the survival time in malignant melanoma patients with exosomal B7-H4 high expression was significantly shorter than that in those with low expression(P<0.05). Moreover, after adjustment for other confounding factors, the multivariate analysis revealed that the exosomeal B7-H4 expression was independent prognostic factors for melanoma patients(P<0.05). Conclusion:A high serum exosomal B7-H4 level in malignant melanoma patients is related to a poor prognosis. B7-H4 may be a potential biomarker of malignant melanoma prognosis.
