血清胃泌素联合MG7-Ag在胃癌前疾病诊断中的临床价值

摘　　要：目的探讨胃癌前疾病患者血清胃泌素及MG7-Ag的水平变化和临床意义。方法本研究中胃癌前疾病主要包括:非萎缩性胃炎、胃溃疡、胃息肉、残胃炎。根据胃镜和组织病理学检查结果将105例胃镜受检者分为4组。(1)对照组:胃黏膜正常或非萎缩性胃炎30例;(2)胃溃疡30例;(3)胃息肉30例;(4)残胃炎15例。血清胃泌素、MG7-Ag检测采用酶联免疫吸附试验(ELISA)方法。结果胃癌前疾病中MG7-Ag在胃溃疡组(57.16±4.68)ng/ml、胃息肉组(42.50±5.21)ng/ml、残胃炎组(44.30±2.99)ng/ml之间差异无统计学意义(P>0.05)。各组分别与对照组(13.27±3.41)ng/ml比较差异有统计学意义(P<0.05)。胃泌素与MG7-Ag联合检查的AUC(0.848)均高于胃泌素与MG7-Ag单独检测,胃泌素浓度可促进胃癌的发生发展,MG7-Ag是一种良好的胃癌标志物,胃泌素与MG7-Ag联合检查对胃癌前疾病诊断的敏感度(81.33%)与特异度(73.33%)较高。结论血清胃泌素联合MG7-Ag测定对胃癌前疾病的预测有指导意义。Objective To investigate the changes and clinical significance of serum gastrin and MG7-Ag levels in patients with gastric precancerous disease.Methods In this study, the precancerous diseases of gastric cancer mainly included non-atrophic gastritis, gastric ulcer, gastric polyp and residual gastritis.According to the results of endoscopy and histopathology, one hundred and five patients were divided into 4 groups.(1)Control group: 30 cases of normal or non-atrophic gastritis of gastric mucosa;(2)30 cases of gastric ulcer;(3)30 cases of gastric polyps;(4)15 cases of residual gastritis.Serum gastrin and MG7-Ag were detected by enzyme-linked immunosorbent assay(ELISA).Results There was no difference in MG7-Ag in gastric ulcer group(57.16±4.68) ng/ml, gastric polyp group(42.50±5.21) ng/ml, gastric residual gastritis group(44.30±2.99) ng/ml(P>0.05).Compared with the control group(13.27±3.41) ng/ml, there was significant difference(P<0.05).The AUC(0.848) of gastrin combined with MG7-Ag was higher than that of gastrin and MG7-Ag alone.The concentration of gastrin can promote the occurrence and development of gastric cancer, and MG7-Ag is a good marker for gastric cancer.The sensitivity(81.33%) and specificity(73.33%) of gastrin combined with MG7-Ag for the diagnosis of precancric cancer were high.Conclusion The determination of serum gastrin combined with MG7-Ag has guiding significance in the prediction of precancillary disease of gastric cancer.
