摘　　要：目的探讨克拉霉素+阿莫西林+奥美拉唑三联疗法联合益生菌对幽门螺杆菌(Hp)感染患儿的临床疗效及对患儿生长发育的影响。方法选取2016年1月至2017年12月河北省儿童医院收治的Hp感染患儿100例为研究对象。按照随机数字表法分为对照组与观察组,各50例。对照组采用克拉霉素+阿莫西林+奥美拉唑三联疗法治疗,观察组在对照组基础上联合益生菌进行治疗,治疗时间均为2周。比较2组临床疗效、Hp根治率、治疗前后患儿发育状况及并发症发生率。结果观察组总有效率高于对照组[92.0%(46/50)比74.0%(37/50)],差异有统计学意义(χ^(2)=4.433,P=0.041)。观察组Hp根治率明显高于对照组[90.0%(45/50)比76.0%(38/50)],差异有统计学意义(χ^(2)=5.129,P=0.034)。2组患儿治疗后身高、体质量、胸围及臀围均明显大于治疗前,且观察组患儿治疗后上述指标水平均明显大于对照组治疗后,差异均有统计学意义(均P<0.05)。治疗期间2组均无严重并发症发生。结论克拉霉素+阿莫西林+奥美拉唑三联疗法联合益生菌对Hp患儿的临床疗效显著,有利于患儿生长发育。Objective To explore the clinical effect of clarithromycin+amoxicillin+omeprazole triple therapy combined with probiotics on children with Helicobacter pylori(Hp)infection and its influence on children′s growth and development.Methods From January 2016 to December 2017,100 children with Hp infection admitted to Hebei Children′s Hospital were selected.They were randomly divided into control group and observation group,with 50 cases in each group.The control group was treated with clarithromycin+amoxicillin+omeprazole triple therapy,and the observation group was treated with probiotics on the basis of the control group.Both groups were treated for 2 weeks.The curative effect,Hp radical cure rate,development status of children before and after treatment and incidence of complications were compared between the two groups.Results The total clinical effective rate of the observation group was significantly higher than that of the control group[92.0%(46/50)vs 74.0%(37/50)](χ^(2)=4.433,P=0.041).The Hp radical cure rate of the observation group was significantly higher than that of the control group[90.0%(45/50)vs 76.0%(38/50)](χ^(2)=5.129,P=0.034).After treatment,the height,body weight,chest circumference and hip circumference of the two groups were significantly higher than those before treatment,and those of the observation group were significantly higher than those of the control group(all P<0.05).No serious complications occurred in both groups during the treatment.Conclusion The triple therapy of clarithromycin+amoxicillin+omeprazole combined with probiotics has a significant clinical effect on children with Hp infection,which is beneficial to the growth and development of children.
