摘 要:目的通过比较直肠癌调强放疗(IMRT)与常规放疗(CRT)照射区体部剂量分布,探讨调强放疗与常规放疗对照射区体部剂量分布的影响。方法选择2018年2月至2021年3月柳州市柳铁中心医院直肠癌术后行IMRT治疗的患者30例,在治疗计划系统中,为每例患者分别设计一套3野CRT计划、一套7野IMRT(7FIMRT)计划。根据每例患者的5、10、15、20、25、30、35、40、45、50 Gy的剂量曲线,计算出其所包含的体部体积V_(5)、V_(10)、V_(15)、V_(20)、V_(25)、V_(30)、V_(35)、V_(40)、V_(45)、V_(50),然后将IMRT计划与CRT计划进行比较。结果30例患者当剂量为5、10、15、20、25、30、35、40、45、50 Gy时的剂量曲线所包含体积均值CRT始终大于7FIMRT,差异有统计学意义(P<0.01)。结论相同的剂量曲线所包含的体积IMRT小于CRT。Objective To compare the dose distribution of irradiated volume between intensity modulated radiotherapy(IMRT)and conventional radiotherapy(CRT)for rectal cancer,and to explore the effect of IMRT and CRT on the dose distribution.Methods From February 2018 to March 2021,30 patients with rectal cancer in Liutie Central Hospital who underwent IMRT after operation were selected.In the treatment planning system,a set of 3-field CRT plan and a set of 7-field IMRT(7 FIMRT)plan were designed for each patient.According to the dose curves of 5,10,15,20,25,30,35,40,45 and 50 Gy of each patient,the body volumes V_(5)、V_(10)、V_(15)、V_(20、V_(25)、V_(30)、V_(35)、V_(40)、V_(45) and V_(50) were calculated,and then the IMRT plan was compared with the CRT plan.Results The mean volume CRT contained in the dose curve of 30 patients at doses of 5,10,15,20,25,30,35,40,45 and 50 Gy was always greater than 7 FIMRT,and the difference was statistically significant(P<0.01).Conclusion The volume IMRT contained in the same dose curve is smaller than CRT.
