摘  要:目的探讨南昌市城市居民胃癌前疾病相关危险因素,为制定胃癌的一级预防措施提供科学依据。方法选取2018-2019年南昌市城市居民胃镜筛查出的144例胃癌前疾病患者为病例组,以同期参加体检的健康体检者288名为对照组,对2组人群进行问卷调查,运用SPSS 24.0进行条件Logistic回归分析。结果单因素分析结果显示,有害物质职业接触、不吃水果、畜肉摄入>7两/周、不吃粗粮、饮食烫、饮食干、饮食稀、饮食口味重盐、饮食油脂高、经常吃腌制品、吸烟、居住环境有人吸烟、饮酒、经常饮茶、精神创伤、精神压抑、高血压和高血脂为胃癌前疾病的危险因素;水果摄入≥2.5斤/周、饮食冷热适中、饮食干稀适中、饮食咸淡适中、饮食油脂适中、偶尔吃腌制品、从不吸烟、从不饮酒、不饮茶和经常参加体育锻炼为胃癌前疾病的保护因素。多因素Logistic逐步回归分析结果显示,有害物质职业接触(农药、煤尘、粉尘,橡胶、石棉等)(OR=6.213,95%CI:2.015~19.160)、饮食烫(OR=8.437,95%CI:2.728~26.090)、经常吃腌制品(OR=10.968,95%CI:2.891~41.620)、饮酒(OR=3.259,95%CI:1.127~9.421)、精神压抑(OR=4.659,95%CI:1.401~15.495)和高血脂(OR=4.285,95%CI:1.588~11.562)为胃癌前疾病的危险因素;饮食干稀适中(OR=0.365,95%CI:0.140~0.954)、饮食油脂适中(OR=0.403,95%CI:0.173~0.943)和从不吸烟(OR=0.287,95%CI:0.100~0.821)为胃癌前疾病的保护因素。结论胃癌前疾病的发生是多因素共同作用的结果,有害物质职业接触、饮食烫、经常吃腌制品、饮酒、精神压抑及高血脂是胃癌前疾病的危险因素,应针对上述因素进行重点干预。Objective To explore the risk factors related to gastric precancerous diseases in urban residents of Nanchang city, and provide scientific basis for formulating primary prevention measures for gastric cancer. Methods Totally 144 patients with gastric precancerous diseases screened by gastroscopy in Nanchang city from 2018 to 2019 were selected as the case group, and 288 people who participated in the physical examination during the same period were selected as the control group. The questionnaire was used to investigate the two groups and SPSS 24.0 was used to perform conditional logistic regression analysis. Results The results of univariate analysis showed that occupational exposure to hazardous substances, no fruit, animal meat intake >7 taels per week, no coarse grains, hot diet, dry diet, thin diet, heavy salt in diet, high fat in diet, frequent pickling food, smoking, living in the place where people smoke, drinking alcohol, drinking tea regularly, mental trauma, mental depression, high blood pressure and hyperlipidemia were risk factors for gastric precancerous diseases.Fruit intake≥1.25 kg/week,moderately hot and cold diet,moderate dry and thin diet,less salt,less grease,occasionally eating pickled products,never smoking or drinking alcohol,not drinking tea and regular physical exercise were protective factors for gastric precancerous diseases.The logistic stepwise regression multivariate analyses showed that occupational exposure to hazardous substances such as pesticides,coal dust,dust,rubber,asbestos,etc.(OR=6.213,95%CI:2.015-19.160),hot food(OR=8.437,95%CI:2.728-26.090),regular consumption of pickled products(OR=10.968,95%CI:2.891-41.620),drinking alcohol(OR=3.259,95%CI:1.127-9.421),mental depression(OR=4.659,95%CI:1.401-15.495)and hyperlipidemia(OR=4.285,95%CI:1.588-11.562)were risk factors for gastric precancerous disease.Moderate dry and thin diet(OR=0.365,95%CI:0.140-0.954),less greasy diet(OR=0.403,95%CI:0.173-0.943)and never smoking(OR=0.287,95%CI:0.100-0.821)were protective factors for gastri
