摘　　要：目的分析中药外敷辅助头孢甲肟治疗阑尾周围脓肿的临床疗效。方法选取阑尾周围脓肿患者100例,以随机数表法分为对照组与观察组,各50例。对照组静脉滴注注射用盐酸头孢甲肟,观察组在对照组治疗基础上予中药自制大黄苏打片药包外敷,2组均治疗6 d。观察2组治疗后疗效,记录2组脓肿消退所用时间、治疗费用及恢复情况,比较2组治疗前后血液指标。结果治疗后,观察组总有效率(92.00%,46/50)高于对照组(76.00%,38/50)(P<0.05)。观察组脓肿消退60%、80%及完全消退时间均短于对照组(P<0.05)。观察组治疗费用低于对照组,体温恢复正常、腹痛消失、恢复进食及住院时间短于对照组(P<0.05)。与治疗前比较,治疗后,2组外周血中性粒细胞比率(NEUT%)、白细胞计数(WBC)及血清C反应蛋白(CRP)水平均降低,且观察组低于对照组(P<0.05)。结论中药自制大黄苏打片药包外敷辅助头孢甲肟治疗阑尾周围脓肿可有效控制患者炎症反应,抑制病情进展,同时可改善患者症状与体征,促进机体恢复,有助于缩短住院时间,降低治疗费用,疗效显著。Objective To analyze the clinical efficacy of the topical application of traditional Chinese medicines combined with cefuroxime in the treatment of periappendiceal abscess.Methods A total of 100 patients with periappendiceal abscess were randomly divided into a control group and an observation group,with 50 cases in each group.The control group was intravenously injected with cefuroxime hydrochloride,while the observation group was externally applied with rhubarb soda tablets on the basis of the control group,both for 6 days.The curative effect of the two groups after treatment was observed,and the time for abscess regression,treatment cost and recovery of the two groups were recorded,and the blood indexes of the two groups before and after treatment were compared.Results After treatment,the total effective rate of the observation group (92.00%,46/50) was higher than that of the control group (76.00%,38/50) (P<0.05).The abscess regression time by 60%,80% and 100% was shorter in the observation group than that in the control group (P<0.05).The treatment cost of the observation group was lower than that of the control group,and the time for body temperature returning to normal,abdominal pain disappearing,the resumption of eating and hospitalization was shorter than that of the control group (P<0.05).After treatment,the levels of the peripheral blood neutrophil ratio (NEUT%),white blood cell count (WBC) and serum C-reactive protein (CRP) in the observation group were lower than those in the control group (P<0.05).Conclusion The external application of self-made rhubarb soda tablets combined with cefmenoxime in the treatment of periappendicial abscess can effectively control inflammatory reactions,inhibit the progress of the disease,improve the symptoms and signs and promote the recovery,which is helpful to shorten the hospitalization time and reduce the treatment cost,with a significant effect.
