针灸联合西药治疗闭角型青光眼继发视神经萎缩的临床疗效观察

摘　　要：目的:观察针灸联合西药治疗闭角型青光眼继发视神经萎缩的临床疗效。方法:选择闭角型青光眼继发视神经萎缩患者100例,采用随机数表法分为对照组和观察组,每组50例。对照组予尼莫地平片治疗。观察组在对照组基础上予针灸治疗。两组均连续治疗4周。观察两组证候积分(主症)、平均光敏感度(mean sensibility,MS)、平均视野缺损(mean deviation,MD)、眼动脉收缩期血流速峰值(peak systolic velocity,PSV)、眼动脉舒张末期血流速度(end diastolic velocity,EDV)变化情况及临床疗效。结果:治疗后,两组证候积分及MS、MD、PSV、EDV均较治疗前改善(P<0.05),且治疗后观察组明显优于对照组(P<0.05);观察组临床总有效率[92.00%(46/50)]高于对照组[66.00%(33/50)]。结论:针灸联合西药治疗闭角型青光眼继发视神经萎缩可有效改善局部血液循环、提高神经兴奋能力、缓解血管痉挛、改善视野、修复视神经,改善视物模糊等不良反应,提高临床有效率。Objective:To explore clinical effects of acupuncture and Western medicine in the treatment of secondary optic atrophy of closed angle glaucoma(CAG).Methods:All 100 patients were chosen,allocated to the control group and the observation group using random number table method,50 cases in each group.The control group was treated by nimodipine tablets,and the observation group by acupuncture on the foundation of Western medicine.Both groups were treated for four weeks consecutively.To observe the changes of TCM syndrome scores(the main symptoms),MS,MD,PSV,EDV and clinical effects in the two groups.Results:After the treatment,TCM syndrome scores,MS,MD,PSV of ophthalmic artery and EDV of both groups improved than these before the treatment(P<0.05),and after the treatment,the observation group was superior to the control group notably(P<0.05);clinical total effective rate of the observation group was 92.00%(46/50),higher than 66.00%(33/50)of the control group.Conclusion:Acupuncture and Western medicine in the treatment of secondary optic atrophy of CAG could effectively improve local blood circulation,raise nerve excitability,alleviate vasospasm,protect visual field,repair optic nerve,improve blurred vision and other,adverse reaction,and lift clinical effective rate.
