摘　　要：目的利用术前增强CT纹理分析及形态学特征评估结肠癌Ki67表达状态。方法选取我院CT平扫及增强图像75例。选取平扫及增强图像病灶最大层面,由2位放射科医师分别绘制ROI并提取纹理分析参数,并同时评估形态学特征。形态学特征采用χ^(2)检验,单组内样本量<5时,采用Fisher精确性检验。采用Shapiro-Wilk test进行正态性检验,应用Mann-Whitney U检验进行不同Ki67状态组间纹理参数差异性检验,通过ROC曲线分析CT纹理参数对预测Ki67的诊断效能。结果Ki67不同表达状态中CT特征没有显著性差异。不同Ki67状态平扫、动脉期及静脉期纹理分析均有显著性差异的参数(P<0.05),其中静脉期两组间有显著性差异的参数最多。ROC分析表明平扫、动脉期及静脉期多个纹理参数在预测结肠癌不同Ki67状态中均具有统计学意义。结论多个CT纹理参数在结肠癌Ki67不同状态组中均有显著性差异,尤其是静脉期参数,CT纹理分析可作为有效预测结肠癌Ki67状态的辅助工具。Objective To predict the Ki67 status of colon cancer using CT texture analysis(TA)and morphological features.Methods The non-contrast enhanced(CE)and CE CT images of 75 patients were analyzed retrospectively.The maximum slice of the lesion in transverse axial images was selected.The ROIs were drawn by two radiologists,respectively.The TA parameters were extracted and morphological features were evaluated.The difference test of morphological features used χ^(2) test,and Fisher accuracy test was used when samples in a single group were less than 5.Shapiro-Wilk test was used for normality test.The difference test of TA parameters between different Ki67 groups used Mann-Whitney U test.The diagnostic performance of TA parameters in predicting Ki67 status was analyzed using receiver operating characteristic(ROC)curve analysis.Results There was no significant difference of morphological features between different Ki67 expression states.There were significant differences in the parameters of non-CE,arterial and venous TA between different Ki67 groups(P<0.05).The parameters in the venous phase were the most.ROC analysis showed that multiple texture parameters in non-CE,arterial and venous phases had statistical significance in differentiating Ki67 groups.Conclusion There are significant differences of multiple CT TA parameters,especially for the venous phase,in different Ki67 groups of colon cancer.CT TA can be used as an auxiliary tool to predict the Ki67 status of colon cancer.
