摘 要:目的探讨隆突性皮肤纤维肉瘤(DFSP)的MRI诊断价值。方法回顾性分析经手术病理证实的17例DFSP的临床及MRI表现。结果17例患者共21个病灶,位置表浅(20个)或深在(1个),边缘清楚(10个)或不清楚(11个),呈单结节(16个)或多结节状(5个);9个病灶突出于皮肤外呈“悬挂征”,8个侵犯邻近皮肤、脂肪或筋膜。病灶T1信号类似肌肉(20个),T2信号均高于肌肉(10个)或与脂肪类似(4个),DWI呈高信号(12个),ADC图为略低信号(9个)。增强扫描动脉期呈不均匀明显强化(4个),动态曲线呈流入型(3个)或平台型(1个),延迟期呈不均匀高信号(8个)。结论MRI能反映DFSP的组织学成分,结合临床表现,可以比较准确地诊断DFSP。Objective To investigate the characteristics of dermatofibrosarcoma protuberans(DFSP)on magnetic resonance imaging(MRI).Methods The MRI and clinical data of 17 patients with histopathologically proved(DFSP)were reviewed.Results Twenty-one Lesions were found in 17 patients,20 of which were superficial and 1 was deep.The lesions were well-defined(n=10)or ill-defined(n=11),singular-nodular(n=16)or multi-lobular(n=5).Nine superficial lesions bulged into the skin surface.Eight lesions infiltrated the adjacent skin,fat and fascia.The lesions were isointense to muscle on T1-weighted imaging(T1WI)(n=20),either hyperintense to musle(n=5)or even isointense to fat(n=4)on T2WI,and showed high signal on DWI(n=12)and low signal on apparent diffusion coefficient(ADC)images(n=9).The time-signal intensity curves(TIC)showed monophasic(n=3)or plateau types(n=1).Eight lesions showed inhomogeneous hyperintense in delayed phase.Conclusion MRI can reflect the histological components of DFSP.Combining MRI features with clinical manifestations,DFSP can be diagnosed more accurately.
