乙癸愈风汤结合针灸治疗缺血性脑卒中恢复期肝肾阴虚证疗效观察

摘　　要：目的探讨乙癸愈风汤联合针灸疗法对缺血性脑卒中恢复期肝肾阴虚证患者的治疗效果,为脑卒中恢复期的临床治疗提供参考。方法采用随机数字表法将80例脑卒中恢复期患者随机分为治疗组与对照组,各40例。对照组采用西医治疗方法,治疗组在西医治疗基础上加用乙癸愈风汤联合针灸治疗,以2周为一个疗程。一个疗程结束后,对2组治疗前后的临床疗效进行评价,美国国立卫生研究所脑卒中评分(NIHSS)评价患者神经功能缺损情况,日常生活活动能力量表(ADL)评估患者生活质量变化,中医症状积分评价患者中医相关症状的改善情况,观察患者血清中碱性成纤维生长因子(BFGF)、血管内皮生长因子(VEGF)水平的变化。结果 2组治疗前各项指标未见统计学差异(P>0.05)。2组患者经治疗后,NIHSS神经功能缺损评分、ADL 生活量表评分、中医相关症状较治疗前均有明显改善,患者血清中BFGF、VEGF 水平较治疗前均明显升高(P<0.05)。治疗组患者总有效率、NIHSS、ADL评分及患者血清中 BFGF、VEGF 的水平均高于对照组,中医症状积分较对照组明显降低,差异具有统计学意义(P<0.05)。结论脑卒中恢复期证属肝肾阴虚的患者在常规西医治疗的同时,联合应用乙癸愈风汤及针灸疗法可有效促进患者神经功能恢复、改善患者生活质量并减轻肝肾阴虚的临床症状,对提高临床疗效具有积极作用。Objective To investigate the therapeutic effect of Yigui Yufeng Decoction combined with acupuncture on ischemic stroke patients with the liver-kidney yin deficiency syndrome in the convalescent phase,and to provide references for its clinical treatment.Methods A total of 80 stroke patients in their convalescent stage were randomly divided into a treatment group and a control group,with 40 cases in each group.The control group was treated with the western medicine,while the treatment group was treated with Yigui Yufeng Decoction combined with acupuncture on the basis of the western medicine treatment,both with a treatment course for two weeks.The clinical efficacy of the two groups before and after treatment was evaluated.The National Institutes of Health Stroke Score (NIHSS) was used to evaluate the patients’ neurological deficits,and the Activity of Daily Living Scale (ADL) was used to evaluate the changes in the quality of life.The TCM symptom score was used to evaluate the improvement of patients’ TCM-related symptoms,and the changes in the serum levels of basic fibroblast growth factor (BFGF) and vascular endothelial growth factor (VEGF) were observed.Results There was no statistically significant difference in the indexes of the two groups before treatment (P>0.05).After treatment,the NIHSS neurological deficit score,ADL life quality scale score,and TCM-related symptom score of the two groups were all significantly improved,and the serum levels of BFGF and VEGF were both significantly higher than those before treatment (P<0.05).The total effective rate,NIHSS score,ADL score,serum levels of BFGF and VEGF in the treatment group were all significantly higher than those in the control group,while the TCM symptom score was significantly lower than that in the control group (P<0.05).Conclusion For the stroke patients with the liver-kidney yin deficiency pattern in their convalescent stage,the combined application of Yigui Yufeng Decoction and acupuncture can effectively promote the recovery of neurological fun
