摘 要:目的探讨胃癌组织中PTEN、RUNX3基因甲基化与幽门螺杆菌感染的关系。方法采用甲基化特异性聚合酶链反应(MSP)检测100例胃癌患者的癌组织及其对应的癌旁组织中PTEN、RUNX3基因启动子甲基化情况。所有受检者行胃镜前均进行^(13)C尿素呼气试验检测幽门螺杆菌感染情况。结果胃癌组织标本中PTEN基因甲基化率高于癌旁组织,差异有统计学意义(40%vs 4%,P<0.01)。RUNX3基因甲基化率高于癌旁组织,差异有统计学意义(64%vs 8%,P<0.01)。胃癌组织中PTEN、RUNX3基因甲基化表达情况与淋巴结转移、TNM分期密切相关(P<0.05),与年龄、性别、肿瘤大小、肿瘤部位无关(P>0.05)。RUNX3基因甲基在低分化程度组织和高中分化程度组织中差异有统计学意义(27.1%vs 61.5%,P=0.001),PTEN基因甲基化在低分化程度组织和中高分化程度组织中差异无统计学意义(43.8%vs 42.3%,P=0.522)。PTEN、RUNX3基因甲基化在H.pylori阳性胃癌组与阴性胃癌组中比较差异有统计学意义(55.9%vs 31.3%,47.1%vs 15.6%,均P<0.05)。结论幽门螺杆菌感染增加抑癌基因PTEN和RUNX3的甲基化,与胃癌的发生发展有关。Objective To investigate the relationships between methylation of PTEN gene,RUNX3 gene and Helicobacter pylori(H.pylori)infection in gastric cancer.Methods Methylation-specific PCR(MSP)was used to detect the methylation status of PTEN gene,RUNX3 gene promoter in DNA from gastric cancer tissue,paracancer tissue samples from 100 gastric cancer patients.^(13)C urea breath test was adopted to test H.pylori infection of all subjects before gastroscope examination.Results The rate of PTEN gene methylation in gastric cancer tissues was statistically higher than that in paracancer tissues(40%vs 4%,P<0.01).The RUNX3 gene methylation rate in gastric cancer tissues was higher than that in paracancer tissues(64%vs 8%,P<0.01).The incidences of PTEN,RUNX3 gene methylation in gastric cancer were correlated with lymphnode metastasis and TNM staging(P<0.05),but not with age,gender,tumor size and tumor site(P>0.05).The RUNX3 gene methylation status was statistically significant between lowly differen-tiated tissue and moderately and highly differentiated tissue(27.1%vs 61.5%,P=0.001).The PTEN gene methylation was not statistically significant between lowly differentiated tissues and moderately and highly differentiated tissue(43.8%vs 42.3%,P=0.522).There was significant difference in PTEN,RUNX3 gene methylation between H.pylori negative gastric cancer group and H.pylori positive gastric cancer group(55.9%vs 31.3%,47.1%vs 15.6%,both P<0.05).Conclusion Helicobacter pylori infection increases the methylation of the tumor suppressor gene PTEN and RUNX3,leading to gastric cancer.
