摘　　要：目的探讨血清细胞角蛋白19片段(cytokeratin 19 fragment,CYFRA21-1)、鳞状细胞癌抗原(squamous cell carcinoma antigen,SCCA)及肿瘤相关抗原125(cancer antigen 125,CA-125)联合检测对肺炎型肺癌的临床诊断价值。方法选择安徽省阜阳市第二人民医院2018年7月至2020年10月收治的87例肺炎型肺癌患者作为肺炎型肺癌组,90例局灶性肺炎患者为肺炎组,回顾性分析两组患者的临床资料,比较两组患者血清CYFRA21-1、SCCA、CA-125水平,分析上述指标联合检测对肺炎型肺癌的诊断价值。结果肺炎型肺癌组患者血清CYFRA21-1、SCCA、CA-125水平均显著高于肺炎组,CYFRA21-1、SCCA、CA-125阳性检出率均显著高于肺炎组,差异有显著性(P<0.05)。血清CYFRA21-1、SCCA、CA-125联合检测肺炎型肺癌的AUC值最大,且诊断敏感度高于单一指标(P<0.05)。结论血清CYFRA21-1、SCCA、CA-125水平对肺炎型肺癌的诊断具有重要参考价值,且联合检测可提高诊断的敏感度及特异度。Objective To study the value of combined detection of serum cytokeratin 19 fragment(CYFRA21-1),squamous cell carcinoma antigen(SCCA),and cancer antigen 125(CA-125)levels in diagnosing pneumonic-type lung cancer(P-ADC).Method This study retrospectively analyzed the clinical data of 87 patients with P-ADC diagnosed and treated in Fuyang Second People’s Hospital between July 2018 and October 2020.Meanwhile,90 patients with focal pneumonia were selected as the pneumonia group.Serum CYFRA21-1,SCCA,and CA-125 levels in all subjects were detected,and the value of the above indicators in combined detection of P-ADC was analyzed.Result The levels and positive detection rates of serum CYFRA21-1,SCCA and CA-125 in the P-ADC group were significantly higher than those in the pneumonia group(P<0.05).The AUC of combination of serum CYFRA21-1,SCCA and CA-125 was the largest for diagnosing P-ADC,and the diagnostic sensitivity was higher than those of each index(P<0.05).Conclusion Serum CYFRA21-1,SCCA and CA-125 levels provide important reference for diagnosing P-ADC.Combined detection can improve the diagnostic sensitivity and specificity.
