摘 要:目的:探究固定床高摆位技术在直肠癌俯卧位碳纤维腹板固定中的应用价值。方法:选取24例直肠癌术后放疗的患者,随机分成A,B两组,每组12例,均采用俯卧位单孔碳纤维腹板固定。A组采用常规摆位方法摆位即体表摆位线与激光线重合,B组采用固定床高摆位方法进行摆位。两组患者第一周前三次,后面每周一次,治疗前和治疗后均行CBCT扫描,取第一周第三次及后面每周一次治疗前CBCT数据及所有治疗后与治疗前CBCT数据差值的绝对值进行统计学分析。结果:A,B组三维方向分次间摆位误差分别为:Vrt(腹背)(5.67±1.49)mm,(1.67±1.18)mm, P<0.05;Lng(头脚)(2.83±1.21)mm,(2.53±0.63)mm, P>0.05;Lat(左右)(1.53±0.94)mm,(1.50±1.01)mm, P>0.05。分次内位移误差分别为:△Vrt(1.23±0.65)mm,(1.12±0.70)mm, P>0.05;△Lng(1.36±0.61)mm,(1.26±0.56)mm, P>0.05;△Lat(1.25±0.79)mm,(1.13±0.51)mm, P>0.05。结论:固定床高摆位能有效减小腹板固定分次间摆位误差,提高体位固定重复性,值得推广。Objective:To explore the application value of fixed bed high positioning technique in the fixation of carbon fiber web in the prone position of rectal cancer.Methods:24 patients with postoperative radiotherapy for rectal cancer were randomly divided into two groups: group A and group B, 12 patients in each group. In group A, the conventional positioning method was used, that is, the body surface positioning line coincided with the laser line, while in group B, the fixed bed height positioning method was used. CBCT scan was performed before and after the first week in the two groups. The absolute value of CBCT data before and after the first week and the difference between all CBCT data after the first week and before the treatment were statistically analyzed.Results:in group A and B, the positioning errors in three-dimensional directions were: Vrt(5.67 ± 1.49) mm,(1.67 ± 1.18) mm, P < 0.05;Lng(2.83 ± 1.21) mm,(2.53 ± 0.63) mm, P > 0.05;lat(left and right)(1.53 ± 0.94) mm,(1.50 ± 1.01) mm, P > 0.05. The internal displacement errors were as follows: △ Vrt(1.23 ± 0.65) mm,(1.12 ± 0.70) mm, P > 0.05;△ Lng(1.36 ± 0.61) mm,(1.26 ± 0.56) mm, P > 0.05;△ lat(1.25 ± 0.79) mm,(1.13 ± 0.51) mm, P > 0.05.Conclusion:fixed bed high positioning can effectively reduce the positioning error of Web fixed times and improve the repeatability of body position fixation, which is worth popularizing.
