HPV E6/E7mRNA联合TCT检测对宫颈癌早期病变筛查诊断效能的影响

摘　　要：目的探讨人乳头瘤病毒(HPV)E6/E7mRNA联合液基薄层细胞学检查(TCT)用于宫颈癌早期病变筛查诊断的价值。方法选取256例接受宫颈癌早期病变筛查患者,均接受HPV E6/E7mRNA检测、TCT检测及病理检查,以病理结果为金标准,分析HPV E6/E7mRNA、TCT单一检测与联合检测对诊断宫颈病变的价值及效能。结果病理结果证实阳性228例(89.06%),正常或炎症28例(10.94%)。经HPV E6/E7mRNA检测阳性210例(82.03%),TCT检测阳性205例(80.08%),联合检测阳性224例(87.50%).HPV E6/E7mRNA联合TCT检测的敏感性、特异性、准确性、阴性预测值均高于单一检测,差异有统计学意义(P<0.05).结论针对宫颈癌早期病变的筛查中,应用HPV E6/E7mRNA联合TCT方法的诊断效能高,值得临床推广与应用。Objective To investigate the value of human papillomavirus(HPV)E6/E7 mRNA combined with liquid based thin layer cytology(TCT)in the screening and diagnosis of early lesions of cervical cancer.Methods 256 patients who were screened for early cervical cancer underwent HPV E6ZE7mRNA detection,TCT detection and pathological examination.Taking the pathological results as the gold standard,the value and efficacy of single and combined detection of HPV E6/E7mRNA and TCT in the diagnosis of cervical lesions were analyzed.Results Pathology confirmed positive in 228 cases(89.06%)and normal or inflammatory in 28 cases(10.94%).210(82.03%)were positive for HPV E6/E7mRNA,205(80.08%)were positive for TCT,and 224(87.50%)were positive for combined detection.The sensitivity,specificity accuracy and negative predictive value of HPV E6/E7mRNA combined with TCT were higher than those of single detection,and the difference was statistically significant(PV0.O5).Conclusion HPV E6ZE7mRNA combined with TCT has high diagnostic efficiency and is worthy of popularization in the screening of early lesions of cervical cancer.
