带蒂颊脂垫修复颊黏膜缺损的动物实验研究

摘　　要：目的:研究带蒂颊脂垫修复兔颊黏膜缺损愈合过程的组织学变化。方法:在新西兰白兔(n=6)双侧颊黏膜造出深至黏膜下层等大的(1 cm×1 cm)缺损,一侧用颊脂垫修复,另一侧自然愈合,分别在1、2、4周进行拍照观察愈合情况,使用Image J对伤口面积进行计算,处死获取标本行组织学观察,行HE及Masson染色,SPSS 23做统计学处理。结果:使用颊脂垫修复创面愈合时间短,黏膜上皮化明显,HE染色结果显示术后1、2、4周与实验组相比,对照组创面上皮化较慢,炎性细胞浸润多。Masson染色显示各时间点实验组胶原纤维较对照组多,1、2周实验组与对照组差异具有统计学意义,随时间延长两组胶原纤维总体均成增长趋势。结论:带蒂颊脂垫修复颊黏膜缺损愈合时间短,上皮化明显,炎症反应更轻,更利于创面愈合。Objective:To study the histological changes in the healing process of buccal mucosa defects in rabbits treated with pedicled buccal fat pad.Methods:D efects of the 1 cm×1 cm and to submucous layer were prepared in bilateral buccal mucosa of New Zealand white rabbits(n=6).One side was repaired with buccal fat pad(test group),while the other side was not treated(control group).At the 1st,2nd and 4th week,images were taken to observe the healing,and the wound area was calculated by Image J.The samples of the wound area were obtained by execution,paraffin sections were made for HE and Masson staining and statistically processed by SPSS 23.Results:The healing time of the wound surface of the test group was shorter and the mucosa epithelialization was more obvious than those of control group.HE staining showed that compared with the test group at the 1st,2nd and 4th week after operation,in control group the wound epithelialization was slower and the infiltration of inflammatory cells was more.Masson staining results showed that there were more collagen fibers in the test group than in the control group at each time point at the first and second week(between the 2 groups,P<0.05).Both groups showed an overall trend of collagen fiber increase over time.Conclusion:Pedicled buccal fat pad transplantation may promote the healing of buccal mucosa defects and the epithelialization and may reduce the wound inflammation.
