自牵引后离断技术在全腹腔镜右半结肠癌根治术中的应用

摘　　要：目的:评价自牵引后离断技术在全腹腔镜右半结肠癌根治术中应用的安全性。方法:回顾分析2018年2月至2021年9月行全腹腔镜右半结肠切除术的75例右半结肠癌患者的临床资料。患者均行全腹腔镜回肠-结肠自牵引后离断Overlap吻合,收集其一般资料、术中情况、术后情况、并发症发生率等。结果:75例患者均顺利完成手术,无中转腹腔外吻合及围手术期死亡病例。手术时间平均(93.4±9.1)min,吻合重建时间(10.3±2.1)min,术中出血量平均(27.5±9.0)mL,术后平均住院(4.9±0.9)d,无吻合口漏等严重并发症发生。其中2例(2.7%)术后出现腹腔感染,经对症治疗后治愈。结论:自牵引后离断技术应用于全腹腔镜右半结肠癌手术安全、可行,值得推广。Objective:To evaluate the safety of self-pulling and latter transection(SPLT)technique in totally laparoscopic right colectomy of colon cancer.Methods:Seventy-five patients with right colon cancer received SPLT-Overlap anastomosis from Feb.2018 to Sep.2021.The general data,surgical and postoperative outcomes,and complication incidence were collected retrospectively.Results:All 75 operations were successful,there was no extracorporeal ileocolic anastomosis and no perioperative mortality.Mean operation duration was(93.4±9.1)min,including(10.3±2.1)min for reconstruction.Mean blood loss was(27.5±9.0)mL,and mean postoperative hospital stay was(4.9±0.9)d.Postoperative complications were found in 2 patients(2.7%),who developed abdominal infection and were cured by the symptomatic treatment.Conclusions:SPLT technique is a safe and feasible procedure in totally laparoscopic right colectomy.It is worthy of promotion.
