摘 要:目的分析牙周炎治疗要点,评价盐酸米诺环素与奥硝唑联合治疗的临床优势。方法120例牙周炎患者,随机分为对照组和观察组,每组60例。对照组采用奥硝唑治疗,观察组采用奥硝唑+盐酸米诺环素软膏治疗。比较两组治疗效果以及治疗前后的炎症指标[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、超敏C-反应蛋白(hs-CRP)]、牙周相关指标(牙龈出血指数、探诊深度)。结果治疗后,观察组治疗总有效率96.67%高于对照组的75.00%,差异具有统计学意义(P<0.05)。治疗后,两组患者IL-6、TNF-α、hs-CRP、牙龈出血指数、探诊深度均低于治疗前,且观察组患者IL-6(7.55±1.60)ng/ml、TNF-α(49.90±6.50)pg/ml、hs-CRP(1.95±0.40)μg/ml、牙龈出血指数(1.56±0.45)、探诊深度(2.89±0.50)mm均低于对照组的(9.80±1.90)ng/ml、(81.55±7.80)pg/ml、(2.70±0.45)μg/ml、(2.20±0.50)、(3.55±0.53)mm,差异均具有统计学意义(P<0.05)。结论盐酸米诺环素联合奥硝唑治疗牙周炎效果理想,可有效改善患者的牙周状态、降低龈沟液中炎症因子水平。Objective To analyze the key points of the treatment of periodontitis and evaluate the clinical advantages of combined treatment with minocycline hydrochloride and ornidazole.Methods A total of 120 patients with periodontitis were randomly divided into control group and observation group,with 60 cases in each group.The control group was treated with ornidazole,and the observation group was treated with ornidazole and minocycline hydrochloride ointment.Both groups were compared in terms of therapeutic effects and inflammation indicators[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),high-sensitivity C-reactive protein(hs-CRP)],periodontal related indicators(gingival bleeding index,probing depth)before and after treatment.Results After treatment,the total effective rate of treatment 96.67% in the observation group was higher than 75.00% in the control group,and the difference was statistically significant(P<0.05).After treatment,the IL-6,TNF-α,hs-CRP,gingival bleeding index,and probing depth of the two groups were lower than those of this group before treatment,and the IL-6(7.55±1.60)ng/ml,TNF-α(49.90±6.50)pg/ml,hs-CRP(1.95±0.40)μg/ml,gingival bleeding index(1.56±0.45),probing depth(2.89±0.50)mm of the observation group were lower than(9.80±1.90)ng/ml,(81.55±7.80)pg/ml,(2.70±0.45)μg/ml,(2.20±0.50),(3.55±0.53)mm of the control group,and the differences were statistically significant(P<0.05).Conclusion Minocycline hydrochloride combined with ornidazole has an ideal effect in the treatment of periodontitis,which can effectively improve the periodontal state of patients and reduce the level of inflammatory factors in gingival crevicular fluid.
