摘　　要：目的比较血管事件总体健康风险(THRIVE)评分、休斯顿动脉内血管再通治疗(HIAT)评分和斯坦福年龄和弥散加权成像(SAD)评分预测接受血管内治疗的缺血性脑血管病患者3个月预后的预测价值。方法选择在同济大学附属同济医院接受血管内治疗的缺血性脑血管病患者152例,按照预后分为预后良好组120例[改良的Rankin量表(mRS)评分≤2分]和预后不良组32例(mRS评分≥3分),分别使用THRIVE评分、HIAT评分和SAD评分预测患者3个月预后。采用ROC曲线分析其预测价值。结果预后不良组THRIVE评分、HIAT评分、SAD评分明显高于预后良好组[4.0(3.3,5.0)分vs 3.0(2.0,4.0)分,1.0(0.0,2.0)分vs 0.0(0.0,1.0)分,3.0(1.3,3.0)分vs 1.0(1.0,2.0)分,P<0.01]。THRIVE评分、HIAT评分和SAD评分3个月预后预测的ROC曲线下面积(AUC)分别为0.803、0.644、0.726(P<0.05),THRIVE评分AUC明显大于HIAT评分(P<0.05)。结论THRIVE评分、HIAT评分和SAD评分对接受血管内治疗的缺血性脑血管病患者的3个月预后有预测价值,THRIVE价值最高。Objective To score,HIAT compare the values of THRIVE score and SAD score for endo-predicting the3-month outcomes in ischemic cerebrovascular disease patients undergoing therapy.Methods One vascular hundred and fifty-two ischemic cerebrovascular disease patients score≤2)outcomes outcome(mRS good undergoing endovascular therapy in our hospital were divided into group(n=120)and score≥3)group(n=32).The outcome(mRS poor3-month and of the score,HIAT patients were predicted according to their THRIVE by score SAD score respectiveanalysis.Results The curve score,ly.Their predictive value was analyzed ROC score THRIVE poor HIAT score and SAD weighted imaging were significantly higher in outcome group vs vs group[4.0(3.3,5.0)3.0(2.0.4.0),1.0(0.0,2.0)0.0(0.0,1.0),than in good outcome vs(1.0,2.0),P<0.01].The1.03.0(1.3,3.0)weighted score,HIAT AUC of THRIVE score and SAD respec-0.803,0.644,and was imaging score for predicting the3-month outcomes was0.726of tively(P<0.05).The AUC of THRIVE score significantly larger than that scores HIAT score predict(P<0.05).Conclusion THRIVE,HIAT and SAD disease weighted imaging undergoing can the therapy,3-month outcomes in ischemic cerebrovascular score patients of endovascular SAD and the value of THRIVE is higher than that HIAT score and weighted imaging score.
