摘  要:目的总结大鼠肝泡球蚴病(HAE)磁共振扩散加权成像(DWI)及超声造影(CEUS)特征,为评估HAE活性状态提供更多影像学指标。材料与方法选择成功感染HAE的SD大鼠31只,同期进行DWI及CEUS检查,比较HAE病灶内部、边缘区、近肝组织及远肝组织的表观扩散系数(ADC);检查结束后处死大鼠,记录病灶数目、最大径、形态及剖面特征,将ADC值与病理结果进行对照。结果病灶内部[(2.8461±0.0741)×10^(-3) mm^(2)/s]与边缘带[(1.6406±0.0732)×10^(-3) mm^(2)/s,t=6.467]、近肝组织[(1.7742±0.1421)×10^(-3) mm^(2)/s,t=6.535]、远肝组织[(1.8968±0.1048)×10^(-3) mm^(2)/s,t=6.467]的ADC值比较,差异均有统计学意义(P均<0.05)。17个病灶(54.8%)CEUS动脉早期出现完整或节段性环状强化(环状强化组),14个(45.2%)全程均未见增强(无强化组)。两组间HAE病灶最大径差异有统计学意义(t=-3.114,P<0.05)。两组间病灶内部(t=1.527)、边缘带(t=-0.233)、近肝组织(t=0.080)及远肝组织(t=0.474)的ADC值比较,差异均无统计学意义(P均>0.05)。环状强化组HAE病灶边缘纤维组织及交界区均可见微血管结构,无强化组HAE病灶边缘纤维组织较为致密,纤维组织内未见微血管结构。结论联合使用CEUS和DWI可以评估HAE病灶的微血流灌注和纤维化状态,为全面评估HAE病灶活性状态提供更多影像学依据。Purpose To summarize the characteristics of contrast-enhanced ultrasonography(CEUS)and diffusion weighted imaging(DWI)of hepatic alveolar echinococcosis(HAE)in rats,in order to evaluate the activity of HAE.Materials and Methods Thirty-one rats with HAE were examined by CEUS and DWI.The maximum diameters and apparent diffusion coefficient(ADC)values of lesions were recorded and compared with the pathological results.Results There was significant difference in ADC values between the lesions[(2.8461±0.0741)×10^(-3) mm^(2)/s]and the marginal zone[(1.6406±0.0732)×10^(-3) mm^(2)/s,t=6.467],and the near hepatic tissue[(1.7742±0.1421)×10^(-3) mm^(2)/s,t=6.535],and the far hepatic tissue[(1.8968±0.1048)×10^(-3) mm^(2)/s,t=6.467](P<0.05).There was significant difference in the maximum diameter of the HAE lesions between the ring enhancement group and the non-enhancement group(t=-3.114,P<0.05).There was no significant difference in ADC values of the lesions(t=1.527,P>0.05),of the marginal zone(t=-0.233,P>0.05),of the near hepatic tissue(t=0.080,P>0.05),of the far hepatic tissue(t=0.474,P>0.05)in different groups.In ring enhanced group,microvascular structures were found in the marginal fibrous tissue and junction area,while in non-enhancement group,no microvascular was found in the marginal dense fibrous tissue.Conclusion The combination of CEUS and DWI can evaluate the microvessel perfusion and fibrosis of HAE lesions,and can provide more imaging features for the comprehensive evaluation of the activity of HAE lesions.
