不同分期肺癌患者血清中PDCD5蛋白、CEA和CYFRA21-1表达水平变化及临床意义

摘　　要：目的探讨不同分期肺癌患者血清中程序化细胞死亡分子5(programmed cell death5,PDCD5)蛋白、癌胚抗原(carcino-embryonic antigen,CEA)和细胞白蛋白19(Cytokeratin-19,CYFRA21-1)表达水平变化及临床意义。方法选取肺癌患者184例作为肺癌组,并选取同期健康体检者80例作为对照组,采用酶联免疫吸附法(ELISA)检测血清PDCD5蛋白水平,电化学发光免疫分析法检测血清CEA和CYFRA21-1水平,比较3种血清指标在2组研究者中的水平及在肺癌不同分期患者中的水平,并采用Pearson分析血清PDCD5蛋白与CEA和CYFRA21-1相关性。结果肺癌组血清PDCD5水平明显低于对照组,CEA和CYFRA21-1水平明显高于对照组,其差异均具统计学意义(P<0.05)。Ⅰ~Ⅱ期肺癌患者血清PDCD5水平明显高于Ⅲ~Ⅳ期肺癌患者,CEA和CYFRA21-1水平明显低于Ⅲ~Ⅳ期肺癌患者,其差异均具统计学意义(P<0.05);经Pearson相关性分析,血清PDCD5水平与肺癌患者TNM分期呈负相关(γ=-0.423),血清CEA和CYFRA21-1水平与肺癌患者TNM分期呈正相关(γ(1)=0.514,γ_(2)=0.346)。结论肺癌患者血清中PDCD5水平存在明显低表达,而CEA和CYFRA21-1水平存在明显高表达,PDCD5、CEA和CYFRA21-1均与TNM分期密切相关,可作为肺癌临床诊断、治疗以及预后评估的参考依据。Objective To explore the changes and clinical significance of serum programmed cell death 5(PDCD5)protein,carcino-embryonic antigen(CEA)and cytokeratin-19(CYFRA21-1)in patients with different stages of lung cancer.Methods 184 patients with lung cancer were enrolled as lung cancer group,while other 80 healthy controls during the same period were enrolled as the control group.The level of serum PDCD5 protein was detected by enzyme-linked immunosorbent assay(ELISA).The levels of serum CEA and CYFRA21-1 were detected by electrochemiluminescence immunoassay.The levels of the 3 serum indexes between the 2 groups and among patients with different stages of lung cancer were compared.The correlation between PDCD5 protein and CEA,CYFRA21-1 was analyzed by Pearson correlation analysis.Results The level of serum PDCD5 in lung cancer group was significantly lower than that in the control group,while the levels of CEA and CYFRA21-1 were significantly higher than those in the control group(P<0.05).The level of serum PDCD5 in patients with lung cancer at stageⅠ-Ⅱwas significantly higher than that at stageⅢ-Ⅳ,while the levels of CEA and CYFRA21-1 were significantly lower than those at stageⅢ-Ⅳ(P<0.05).Pearson correlation analysis showed that level of serum PDCD5 was negatively correlated with TNM staging of lung cancer(γ=-0.423),while levels of serum CEA and CYFRA21-1 were positively correlated with TNM staging(γ_(1)=0.514,γ_(2)=0.346).Conclusion The level of serum PDCD5 is significantly lower,while levels of serum CEA and CYFRA21-1 are significantly higher in lung cancer patients.PDCD5,CEA and CYFRA21-1 are closely related to TNM staging,which can be applied in the clinical diagnosis,treatment and prognosis evaluation of lung cancer.
