摘　　要：目的探究行肩胛舌骨肌上颈淋巴结清扫术对cN0N1期口腔鳞癌患者术后生活质量的影响。方法纳入口腔鳞癌患者112例,依据手术方法不同将行功能性颈淋巴结清扫术(FND)的62例患者纳入FND组,将行肩胛舌骨肌上颈淋巴结清扫术(SOND)的50例患者纳入SOND组。对比2组术后肩关节功能恢复情况、手术前后生活质量评分、术后1年生存率及转移率。结果SOND组术后1个月及3个月肩部疼痛评分均低于FND组,肩部外展角度均大于FND组,差异有统计学意义(P<0.05)。SOND组术后生活质量各项评分均高于FND组,差异有统计学意义(P<0.05)。2组术后1年生存率及转移率比较,差异无统计学意义(P>0.05)。结论行SODN治疗的cN0N1期口腔鳞癌患者术后肩部疼痛感较轻,肩关节活动不受影响,术后生活质量改善明显,复发率及转移率较低。Objective To investigate the effect of superior scapular tongue muscle cervical lymph node dissection on postoperative quality of life in patients with stage cN0N1 oral squamous carcinoma.Methods 112 patients with oral squamous carcinoma were included,62 patients treated with functional cervical lymph node dissection(FND)were included in the FND group,and 50 patients with superior scapular tongue muscle cervical lymph node dissection(SOND)were included in the SOND group.The recovery of shoulder function,quality of life score before and after surgery,postoperative survival rate and metastasis rate of the 2 groups were compared.Results The shoulder pain score was lower at 1 and 3 months than FND group and the shoulder abduction angle was greater than FND group(P<0.05).The SOND group had higher postoperative QOL scores than the FND group(P<0.05).Survival rates and transfer rates of the 2 groups had no statistically significant difference(P>0.05).Conclusions Patients with stage cN0N1 oral squamous carcinoma treated with SODN have mild postoperative shoulder pain,unaffected shoulder activity,significant postoperative quality of life improvement,and low recurrence rate and metastasis rate.
