摘 要:目的探讨不同强度有氧运动对脑胶质瘤术后化疗患者癌因性疲乏(CRF)及下丘脑-垂体-肾上腺皮质(HPA)轴功能的影响。方法选取脑胶质瘤术后化疗并发CRF患者76例,随机分为2组,各38例。A组予以25%VO2max的强度进行有氧运动,B组予以50%VO2max的强度进行有氧运动,2组均连用8周。统计分析2组治疗前后HPA轴功能[血清促肾上腺皮质激素(ACTH)和皮质醇(CORT)]和Piper疲乏量表评分的变化,并比较其临床疗效。结果治疗8周后,2组血清ACTH较前明显降低,CORT水平较前明显提升(P<0.05或P<0.01),且B组降低或提升幅度明显优于A组(P<0.05)。2组Piper疲乏评分较前明显减低(P<0.05或P<0.01),且B组降低明显优于A组(P<0.05)。B组患者临床总有效率(94.74%)高于A组(78.95%)(χ^(2)=4.040,P<0.05)。结论50%VO2max的有氧运动对脑胶质瘤术后化疗患者患者CRF及HPA轴的影响明显优于25%VO2max的有氧运动。Objective To explore the effects of different intensities of aerobic exercise on cancer-related fatigue(CRF)and the function of the hypothalamic-pituitary-adrenal cortex(HPA)axis in patients undergoing chemotherapy after glioma surgery.Methods 76 patients with glioma who had gone through postoperative chemotherapy and CRF were selected and randomly divided into 2 groups,with 38 cases in each group.The patients in Group A was given aerobic exercise with 25%VO2max,and the patients in group B was given aerobic exercise with 50%VO2max.The patients in the 2 groups were treated for 8 weeks.The changes of HPA axis function[serum adrenocorticotropic hormone(ACTH)and cortisol(CORT)]and Piper fatigue scale scores before and after treatment were statistically analyzed in the 2 groups,and their clinical effects were compared.Results After 8 weeks'treatment,the serum ACTH of patients in the 2 groups were significantly lower than before,and the level of CORT was significantly were increased than before(P<0.05 or P<0.01),and the decrease or increase in group B was significantly better than that of group A(P<0.05);The Piper fatigue score of patients in the 2 groups was significantly lower than before(P<0.05 or P<0.01),and the reduction in group B was significantly better than that of group A(P<0.05);at the same time,the total clinical effective rate of patients in group B(94.74%)was higher than that of group A(78.95%)(χ^(2)=4.040,P<0.05).Conclusion Aerobic exercise with 50%VO2max has significantly better effect on CRF and HPA axis in patients with postoperative chemotherapy for glioma than aerobic exercise with 25%VO2max.
