摘  要:目的探讨基于低频振荡振幅(amplitude of low frequency fluctuation,ALFF)的静息态功能磁共振成像(resting state functional magnetic resonance imaging,rs-fMRI)在伴中央颞区棘波儿童良性癫痫(benign childhood epilepsy with centrotemporal spikes,BECTS)异常放电脑区脑活动状态及抗癫痫治疗作用评价中的应用价值。方法根据是否接受抗癫痫药物(antiepileptic drugs,AEDs)治疗将56例BECTS分为治疗组(n=28例)、未治疗组(n=28例),另选取同期28例健康正常儿童作为正常对照组。采用西门子3.0T超导磁共振扫描仪采集三组受试者rs-fMRI数据,经数据预处理后应用SPM8软件的方差分析比较三组受试者各脑区ALFF。结果未治疗组双侧中央前/后回、双侧脑岛、双侧额上回、右侧前扣带回ALFF较正常对照组增加,而左侧颞中回、左侧枕中回、左侧后扣带回、双侧楔叶ALFF较正常对照组减少,差异有统计学意义(P<0.05)。治疗组双侧丘脑、双侧中央前/后回、双侧额上回、右侧颞上回、右侧梭状回、右侧前扣带回ALFF较正常对照组增加,差异有统计学意义(P<0.05)。治疗组双侧中央前/后回、双侧前脑岛ALFF较未治疗组减少,差异有统计学意义(P<0.05)。结论基于ALFF的rs-fMRI不仅有助于反映BECTS异常放电脑区脑活动状态,还可评价AEDs治疗后BECTS脑功能的改变情况。Objective To investigate the application of resting state functional magnetic resonance imaging(rs-fMRI)with amplitude of low frequency fluctuation(ALFF)in evaluation of brain activity and antiepileptic therapy of benign childhood epilepsy with centrotemporal spikes(BECTS).Methods 56 patients with BECTS were divided into treatment group(n=28)and untreated group(n=28)according to whether they received antiepileptic drugs(AEDs).Another 28 healthy children were selected as control group.The rs-fMRI data of three groups were collected by Siemens 3.0 T superconducting MRI scanner.After data preprocessing,the ALFF of each brain region was compared among the three groups by using the analysis of variance(ANOVA)of SPM8 software.Results The ALFF of bilateral anterior/posterior gyrus,bilateral insula,bilateral superior frontal gyrus and right anterior cingulate gyrus in untreated group were higher than those in normal control group,while ALFF in left middle temporal gyrus,left middle occipital gyrus,left posterior cingulate gyrus and bilateral cuneiform lobe in untreated group were lower than those of normal control group significantly(P<0.05).The ALFF of bilateral thalamus,bilateral anterior/posterior central gyrus,bilateral superior frontal gyrus,right superior temporal gyrus,right fusiform gyrus and right anterior cingulate gyrus in the treatment group were significantly higher than those of normal control group significantly(P<0.05).The ALFF of bilateral anterior/posterior gyrus and bilateral anterior insula in the treatment group was significantly lower than that of control group significantly(P<0.05).Conclusion rs-fMRI with ALFF is helpful to reflect the brain activity of BECTS,and evaluate the changes of brain function after AEDs treatment.
